Achillon缝合器与改良Kessler法治疗急性跟腱断裂的效果比较

摘　　要：目的：比较Achillon缝合器与改良Kessler缝合法治疗新鲜跟腱断裂的临床效果。方法：回顾性分析采用改良Kessler缝合法（改良Kessler组）或Achillon缝合器（ Achillon组）治疗的32例新鲜跟腱断裂患者的临床资料,从术前评估、术中情况、术后并发症及疗效等方面对两种治疗方法的临床效果进行比较。结果：所有患者经过9～17个月随访,平均（12.3＆#177;1.5）个月,随访率l00％。两组患者手术时间比较差异无统计学意义（ P〉0.05）,Achillon组切口长度、术中出血量、住院时间、愈合时间、并发症总发生率均低于改良Kessler组（均 P〈0.05）；根据 Amer-Lindholm踝关节评分标准,Achillon组术后优良率高于改良Kessler组（ P〈0.05）。结论：Achillon缝合器治疗新鲜跟腱断裂具有创伤小、术后并发症少及功能恢复好等特点,是治疗新鲜跟腱断裂的理想装置。Objective：To investigate the clinical effect of Achillon stiching instrument versus modified Kessler suture in the treatment of acute Achilles tendon rupture. Methods：The clinical data of 32 patients with acute rupture of Achilles tendon treated by Kessler suture （ modified Kessler group ） or Achillon stiching instrument （ Achillon group） were retrospectively analyzed. The preoperative evaluation, intraoperative condition, and postoperative complications and clinical effects between the two treatment methods were evaluated. Results：All patients were followed up for 9～17 months, with an average of 12.3 months, and the follow-up rate of 100%. There was no statistically significant difference in the operation time between the two groups （ P〉0.05）;whereas the incision length, intraoperative blood loss, hospitalization time, healing time and the total incidence of complications between the two groups （ P〈0.05） . The total incidence of complications in the Achillon group was lower than that in the modified Kessler group. According to the Amer-Lindholm ankle joint scoring standard, there was statistically significant difference in the postoperative excellent rate between the two groups （P〈0.05）. The postoperative excellent rate in the Achillon group was higher than that in the modified Kessler group. Conclusion：Achillon stiching instrument has the characteristics of less trauma, fewer complications and good functional recovery in the treatment of acute Achilles tendon rupture. It is an ideal device for the treatment of acute Achilles tendon rupture.
