摘 要:目的:分析施行《抗菌药物临床应用管理办法》限制抗菌药物品种、品规政策对医院抗菌药物消耗量的影响。方法:回顾性统计我院经抗菌药物目录调整期间(2011~2014年),抗菌药物的品种数量、品规数量、销售金额、用药频度随时间阶段的变化情况。结果:抗菌药物管理政策落实后,抗菌药物品种数从2011年的97种降至2012年的49种,品规数从2011年的255个降至2012年的84个。2011年销售前10品种中的特殊使用级、限制级药品在2012年的前10品种中使用份额显著下降,2011年使用频度最高的三代头孢菌素头孢地嗪(DDDs值=11 2625)随后被一代头孢(头孢唑啉或头孢硫脒)所取代;2011年排名前10品规占抗菌药物销售金额比例最低25.74%,随后3年逐年递增,依次为54.63%、60.87%、56.97%。结论:《抗菌药物临床应用管理办法》实施以来,我院抗菌药物使用结构、数量及金额发生显著改变,有助于药品规范化管理及临床合理应用。Objective: To analyze the effects on antibacterial agent consumption by variety and specification restriction according to the relative requirements in Administrative Measures for the Clinical Use of Antibacterial Drugs. Methods: A retrospective study was carried out,and the variety,specification,sale amounts and use density of antibacterial agents during the antimicrobial agent directory adjustment in our hospital( 2011-2014) were quantitatively recorded and analyzed by statistical methods. Results: After the policy implementation,the variety was decreased from 97 in 2011 to 49 in 2012,and the specification was decreased from 255 to 84. The proportion of sale amounts of special use and restricted use antibacterial agents in the top 10 in 2012 was significantly decreased when compared with that in 2011,and the third generation cephalosporin( cefodizime,DDDs = 112 625) most frequently used in 2011 was replaced by the first generation cephalosporin( cefazolin and cefathiamidine). The proportion of sale amounts of antibacterial agents in the top 10 was 25. 74% in 2011,which was the lowest one,and that was increased year by year,which was 54. 63% in 2012,60. 87% in 2013 and 56. 97% in 2014. Conclusion: The quantity and distribution of variety and specification of antibacterial agents are significantly changed after the implementation of Administrative Measures for the Clinical Use of Antibacterial Drugs,which is beneficial to the rational use of antibacterial agents.
