摘  要:目的研究石家庄市2011—2014年食品及食物中毒分离的129株奇异变形杆菌(P.mirabilis)耐药及基因分子特征。方法选取BD Phoenix全自动细菌鉴定/药敏系统初筛可疑产β-内酰胺酶的奇异变形杆菌菌株,用PCR方法检测超广谱β-内酰胺酶基因(bla TEM、bla SHV、bla CTX-M-1、bla CTX-M-2、bla CTX-M-9、bla CTX-M-13)、Amp C基因(bla DHA、bla CMY)和碳青霉烯酶基因(bla VIM、bla IMP、bla OXA),并对其进行测序。结果 116株(占89.9%)奇异变形杆菌检出耐药基因,经DNA测序后最常见基因型为bla OXA-320(n=102),其次是bla CTX-M-14(n=61)、bla TEM-1(n=36)和bla CMY-2(n=11),其他耐药基因未检出。27株(占20.9%)奇异变形杆菌携带单一耐药基因,89株(占69.0%)携带两种及以上耐药基因。菌株ESBLs流行模式主要为CTX-M-14+OXA-320(占38.8%),其次是TEM-1+OXA-320(占18.6%),再次为OXA-320(占14.7%)。结论首次在变形杆菌中检出OXA-320型碳青霉烯酶,变形杆菌ESBL流行模式为CTX-M-14+OXA-320,单菌株可同时存在多种耐药机制。Objective To study the drug resistance gene molecular characteristics of 129 strains Proteus mirabilis(P. mirabilis)isolated from food and food poisoning related samples in 2011 to 2014. Methods P. mirabilis strains producing β-lactamase were screened and extended spectrum β- lactamase gene(bla TEM, bla SHV, bla CTX-M-1 group, bla CTX-M-2 group, bla CTX-M-9 group,bla CTX-M-13 group), Amp C gene(bla DHA, bla CMY) and carbapenemase gene(bla VIM, bla IMP, bla OXA) were detected by PCR, and the products were sequenced. Results A total of 116 stains(89.9%) of P. mirabilis were detected with resistance gene. The sequencing results showed the most common genotype was bla OXA-320(n=102), followed by bla CTX-M-14(n=61), bla TEM-1(n=36) and bla CMY-2(n=11). The remaining resistance gene were not detected. A total of 27 strains(20.9%) of P. mirabilis carried a single resistance gene, 89 stains(69.0%) carried two or more resistance genes.The mainly ESBL popular mode was CTX-M-14+ OXA-320(38.8%) in Proteus, followed by TEM-1+OXA-320(18.6%), the third was OXA-320(14.7%). Conclusion This is the first report of OXA-320 type carbapenemase β-lactamase resistance. ESBL popular mode is CTX-M-14+OXA-320 in Proteus.Variety of drug-resistant gene may exist in single strains simultaneously.
