摘　　要：目的探讨血清脂联素水平与2型糖尿病(type 2 diabetes mellitus,T2DM)患者糖尿病视网膜病变(diabetic retinopathy,DR)的相关性。方法收集T2DM住院患者165例,根据眼底检查分为正常眼底(nonDR,NDR)组83例和DRP组82例,同期正常健康人群82例作为正常对照(normal control,NC)组。收集病史并检测各组血清中脂联素水平及相关生化指标。结果与NC组比较,NDR组和DR组糖化血红蛋白、空腹血糖、餐后2小时血糖、血肌酐、胱抑素C明显升高(P<0.05),血清脂联素水平明显下降(P<0.05)。与NDR组比较,DR组血清脂联素水平明显下降,病程、胱抑素C、餐后2小时血糖升高明显(P<0.05)。二元Logistic回归结果显示低血清脂联素水平及糖尿病病程是T2DM合并视网膜病变的危险因素。结论血清脂联素水平与T2DM合并视网膜病变呈负相关。Objective To investigate the correlation of adiponectin concentration and diabetic retinopathy （DR） in patients with type 2 diabetes mellitus （T2DM）. Methods We collected 165 T2DM cases and divided them into 83 cases of non-diatetic retinopathy （NDR） group and 82 cases of DR group according to ocular fundus examination.Another 82 healthy individuals （age and gender matched） were recruited as the normal control group （NC）.We collected their medical history,adiponectin concentration and other related biochemical indicators. Results Compared with the NC group,glycosylated hemoglobin A1c,fasting blood glucose,2 h postprandial plasma glucose,serum creatinine,cystatin C were significantly higher in DR and NDR groups （P〈0.05）,and the concentration of serum adiponectin was significantly decreased （P〈0.05）.Compared with NDR group,the concentration of serum adiponectin was significantly decreased, and the course of disease,2 h postprandial plasma glucose and cystatin C were significantly higher in DR group （P〈0.05）.Binary Logistic regression results showed that the serum adiponectin concentration and disease course were the risk factors for type 2 diabetic retinopathy. Conclusions The concentration of serum adiponectin and DR of T2DM is negatively correlated.
