替加环素对复杂性腹内感染患者的疗效及安全性

摘　　要：目的探讨替加环素对复杂性腹内感染患者的临床疗效及安全性。方法选取宁波市第七医院ICU收治的复杂性腹腔感染患者52例,按随机数字表法分组,对照组26例给予常规抗感染治疗;研究组26例在常规抗感染基础上加用替加环素,治疗前后均空腹抽取肘静脉血5 m L,测定CD分子及NK细胞水平,血清乳酸及降钙素原水平,同时对比临床疗效及安全性。结果对照组有效率69.23%低于研究组有效率92.31%,差异具有统计学意义(P<0.05);2组间治疗后免疫功能及降钙素原、血清乳酸水平均发生变化,研究组治疗后CD3^+、CD4^+及CD4^+/CD8^+、NK细胞水平高于对照组,CD8^+水平低于对照组,治疗后降钙素原及血清乳酸水平低于对照组,差异具有统计学意义(P<0.05),2组间不良反应差异无统计学意义。结论替加环素能提高复杂性腹内感染的治疗效果,降低乳酸及降钙素原水平,安全性高。Objective To investigate the clinical effect and safety of tigecycline on in patients with complicated intra-abdominal infections.Methods 52 cases of patients with complicated intra-abdominal infection were selected and randomly divided into 2 groups. 26 cases in the control group were given routine anti-infection treatment,and 26 cases in the treatment group were treated with conventional anti-infection combined with tigecycline. The CD molecules and NK cell level,serum lactate and procalcitonin levels,and the clinical efficacy and safety were compared after the treatment between two groups. Results The effective rate of control group（ 69. 23%） was lower than the treatment group（ 92. 31%） with statistical significance（ P〈0. 05）,the levels of CD3^＋,CD4^＋/ CD8^＋,CD4^＋and NK cells of the treatment group after treatment were higher than the control group,the level of CD8^＋was lower than the control group after treatment,procalcitonin and serum lactate levels were lower than the control group,with statistical significance（ P〈0. 05）,there was no difference in adverse reactions between the two groups. Conclusion Tigecycline could improve the therapeutic effect of complicated intra-abdominal infections,reduce the lactate and procalcitonin levels with high safety.
