玉丹荣心丸治疗急性心肌梗死的疗效及可能机制

摘　　要：目的探讨玉丹荣心丸治疗急性心肌梗死（acute myocardial infarction,AMI）的临床效果,并研究其可能机制。方法选取2014年2月～2015年2月大庆油田总医院心内科收治的86例AMI患者,随机数字表法分为观察组和对照组,各43例。对照组采用常规药物治疗,观察组在对照组的基础上加用玉丹荣心丸,比较2组患者治疗前后炎性相关因子、心肌损伤指标和心功能指标。结果 2组治疗后血清炎性相关因子超敏C反应蛋白（hs-CRP）、白细胞介素6（IL-6）、可溶性血管细胞黏附分子（s VCAM-1）、肿瘤坏死因子α（TNF-α）、P选择素（P-selectin）明显降低,心肌损伤指标肌酸激酶（CK-MB）、乳酸脱氢酶（LD）、心肌肌钙蛋白I（c Tn I）、肌红蛋白（Mb）也明显降低,心功能指标左室收缩末期容积（LVESV）、左室舒张末期容积（LVEDV）均降低,左室射血分数（LVEF）增高,与治疗前相比差异有统计学意义（P〈0.05）。与对照组相比,观察组治疗后hs-CRP、IL-6、s VCAM-1、TNF-α、P-selectin、CK-MB、LD、c Tn I、Mb降低程度优于对照组（P〈0.05）;心功能改善的程度优于对照组（P〈0.05）。结论玉丹荣心丸用于治疗AMI能较好地改善心脏功能,其作用可能与抑制炎性因子以及保护心肌免受损伤有关。Objective To investigate clinical effect and mechanism of Yudanrongxin pills in the treatment of patients with acute myocardial infarction（ AMI）. Methods 86 cases of AMI in our hospital from February 2014 to February 2015 were selected and divided into the observation group and the control group,with 43 cases in each group. The patients in the control group were treated with conventional therapy,while patients in observation group were treated with Yudanrongxin pills on the basis of the control group. The related inflammation factors,indicators of myocardial injury and heart function index were compared between two groups before and after treatment. Results Compared with before treatment,in both two groups after treatment,the serum inflammatory factors including high-sensitivity C-reactive protein（ hs-CRP）,IL-6,soluble vascular cellular adhesion molecule-1（ s VCAM-1）,TNF-α and P-selectin decreased,the myocardial injury criterion including creatine kinase-MB（ CK-MB）,lactate dehydrogenase（ LD）,myocardial troponin I（ c Tn I） and myoglobin（ Mb） decreased,the cardiac function indexes of left ventricular end systolic volume（ LVESV） and left ventricular end diastolic volume（ LVEDV） decreased and left ventricular ejection fraction（ LVEF） increased（ P〈0. 05）. Compared with control group,the hs-CRP,IL-6,s VCAM-1,TNF-α,P-selectin,CK-MB,LD,c Tn I and Mb in observation group were lower（ P〈0. 05）. The degree of improvement at cardiac function was better than that in control group（ P〈0. 05）. Conclusion Yudanrongxin pills could better improve cardiac function in treatment with AMI,its role was relative to inhibition of inflammatory factors and myocardial protection against injury.
