耳穴压豆配合五行音乐疗法防治机械通气患者发生谵妄的疗效观察

摘　　要：目的探讨耳穴压豆配合五行音乐疗法防治机械通气患者发生谵妄的临床疗效。方法 2014年6月至2015年12月,便利抽样法选取在广州中医药大学第一附属医院重症医学科(intensive care unit,ICU)机械通气治疗的患者136例为研究对象,按随机数字表法将其分为观察组与对照组,每组68例。对照组患者使用常规护理,观察组患者除常规护理外,加上耳穴压豆配合五行音乐疗法。治疗7 d后,采用躁动-镇静评分量表(the Richmond agitation sedation scale,RASS)评估两组患者的意识水平或镇静水平,并联合中文版ICU谵妄评估量表(ICU delirium assessment scale,CAM-ICU)评价两组患者是否发生谵妄。比较两组患者咪达唑仑的用量、使用呼吸机时数、平均住院天数、日均医疗费用等。结果对照组患者谵妄发生率为70.6%,高于观察组患者的35.3%,差异有统计学意义(P<0.05)。两组患者住院期间咪达唑仑总用量、使用呼吸机时数的差异均有统计学意义(均P<0.05)。结论耳穴压豆配合五行音乐疗法用于防治ICU谵妄疗效显著,值得临床推广应用。Objective To discuss the clinical effect of ear pressure beans combining with five lines of music therapy in prevention and treatment of delirium in ICU.Methods By convenience sampling,136 cases were selected and randomly divided into observation group and control group,with 68 cases in each group.The control group applied with conventional nursing, and the observation group applied with ear pressure beans combining with five lines of music therapy on the basis of conventional method.After 7 days of treatment, the incidence of delirium was assessed by the Richmond agitation sedation scale （RASS）,ICU delirium as- sessment scale assessment scale （CAM）-the ICU.The dosage of midazolam, mechanical ventilation time, the average hospitalization days, average daily medical expenses were compared between two groups.Results The incidence of delirium in control group（70.6%） was higher than observation group of 35.3% （P〈 0.05）.The statistical significance was found between dosage of midazolam and ventilation time （all P〈 0.05）.Conclusions The clinical effect of ear pressure beans combining with five lines of music therapy in prevention and treatment of delirium in ICU is significant,it is worthy of clinical application.
