输尿管硬镜联合软镜钬激光碎石术治疗复杂上尿路结石的临床研究

摘　　要：目的探讨输尿管硬镜联合软镜钬激光碎石术治疗复杂上尿路结石的临床效果。方法方便选取2011年5月—2015年6月在该院接受治疗的60例上尿路结石患者作为研究对象。对患者进行随机分组,对照组30例给予输尿管硬镜联合体外碎石术治疗,观察组30例给予硬镜联合软镜钬激光碎石术治疗,对比分析两组患者的碎石成功率、复发率及手术情况。结果经治疗,观察组排石率为96.67%,对照组为70.00%,观察组明显高于对照组;观察组患者碎石成功率为93.33%,对照组为73.33%,观察组显著高于对照组;复发率明显低于对照组,两组间对比差异有统计学意义(P<0.05)。观察组平均手术时间、住院时间、出血量及排石时间均明显少于对照组,比较差异有统计学意义(P<0.01)。结论输尿管硬镜联合软镜钬激光碎石术治疗复杂上尿路结石的效果明显优于硬镜钬激光碎石术,值得临床推广及应用。Objective To investigate the clinical effects of ureteroscopic joint soft holmium laser lithotripsy surgery treat-ment of complex upper urinary tract calculi. Methods Convenient selection 60 cases of patients with upper urinary tract cal-culi treated in our hospital from May 2011 to June 2015 were selected for the study.Patients were randomized grouped, the control group of 30 patients were given ureter hard lens combined with lithotripsy surgery for treatment,the observation group of 30 patients were given soft hard endoscopic holmium laser lithotripsy treatment, the lithotripsy success rate, recur-rence rate and surgical cases of two groups of patients were comparatively analyzed. Results After treatment, lithotripsy suc-cess rate of patients in the observation group was 93.33%,the control group was 73.33%, the observation group was signifi-cantly higher than the control group, recurrence rate was significantly lower than the control group,comparison between two groups were significantly different （P〈0.05）. Mean operative time, hospital stay , bleeding and excluding stones time of the observation group were significantly less than the control group, the difference was significant （P〈0.01）. Conclusion The clinical effect of ureteroscopic joint soft holmium laser lithotripsy surgery treatment of complex upper urinary tract calculi is significantly better than hard holmium laser lithotripsy surgery, worthy of clinical promotion and application.
