摘 要:目的探讨早期血液灌流串联透析对急性百草枯中毒患者血清丙二醛(MDA)和超氧化物歧化酶(SOD)的影响。方法将98例急性百草枯中毒患者随机分为实验组和对照组各49例,对照组给予单纯血液灌流治疗,实验组给予血液灌流串联透析。比较治疗前后血清MDA和SOD水平的变化,脏器损伤以及死亡率。结果治疗前两组患者血清MDA和SOD水平差异无统计学意义(P>0.05),治疗后均显著改善(P<0.05),实验组显著优于对照组(P<0.05);治疗前两组患者丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、肌酸激酶(CK)、乳酸脱氢酶(LDH)水平差异无统计学意义(P>0.05),治疗后均显著降低(P<0.05),实验组显著低于对照组(P<0.05);实验组死亡率(24.49%)显著低于对照组(53.06%),差异有统计学意义(P<0.05)。结论血液灌流串联透析可有效减轻急性百草枯中毒患者脏器损伤,降低死亡率,可能与改善MDA、SOD水平有关。Objective To explore the effects of early blood perfusion combined with dialysis on the levels of serum malondialdehyde(MDA) and superoxide dismutase(SOD) in patients with acute paraquat poisoning. Methods Ninetyeight cases of patients with acute paraquat poisoning were randomly divided into experimental group and control group,with 49 cases in each group.The control group was given single blood perfusion,while the experimental group was given early blood perfusion combined with dialysis. The changes of the levels of serum MDA and SOD before and after treatment,the degrees of organ damageand the mortality rates of the two groups were compared. Results The differences in the serum MDA and SOD levels between the two groups before treatment were not statistically significant(P 〈0.05),which were significantly improved after treatment,and the levels of serum MDA and SOD in the experimental group were significantly better than those in the control group(P 〈0.05). There were no significant differences in the levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),creatine kinase(CK) and lactate dehydrogenase(LDH)between the two groups before treatment(P 〈0.05),which were significantly decreased after treatment(P 〈0.05),of which the experimental group were significantly lower than those of the control group(P 〈0.05).The mortality rate of the experimental group was significantly lower than that of the control group(P 〈0.05). Conclusion Blood perfusion combined with dialysis can effectively reduce the organ damage of patients with acute paraquat poisoning and reduce mortality,which may be related to improving the levels of serum MDA and SOD.
