不同赋形剂调制金黄散干预蝮蛇咬伤肢体肿痛的疗效比较

摘　　要：目的比较不同赋形剂调制金黄散干预蝮蛇咬伤肢体肿痛的效果。方法将2014年5月—2016年1月符合纳入标准的60例蝮蛇咬伤患者随机分为酒精组、蜂蜜组和醋组。各组均给予常规治疗方案,包括局部处理、全身治疗、中药金黄散外敷,不同之处在于3组金黄散采取的赋形剂分别为75%酒精（酒精组）、蜂蜜（蜂蜜组）和醋（醋组）。分别于干预第3天、第7天评价患肢周径及疼痛评分,同时统计患肢肿胀消退总时间及不良发应率。结果蜂蜜组和醋组在干预第3天、第7天,患肢周径及疼痛评分均显著低于酒精组（P=0.043,0.048,0.010,0.030）;在患肢肿胀完全消退方面,蜂蜜组、醋组肿胀消退总时间均显著少于酒精组（F=3.247,P=0.046）;在安全性方面,以蜂蜜调制的金黄散组不良反应发生率明显低于以醋或以酒精调敷（χ^2=7.247,P=0.024）。结论在蝮蛇咬伤肢体肿痛局部箍围治疗期间,以蜂蜜调制的金黄散其疗效及安全性俱佳。Objective To explore the effect of Ru Yi Jin Huang San mixed with different excipients on limb swelling from snakebite.Methods From May 2014 to January 2016, a total of 60 patients selected were randomly divided into control group, intervention group I and intervention group II. Each group was given conventional treatments, including local treatment, systemic therapy, and external application with Jin Huang San but different excipients including 75% alcohol, honey and vinegar were applied in control group, intervention group I and intervention group II respectively. The affected limb circumference, pain scores, limb swelling overall time and the rate of adverse were compared respectively on 3rd and 7th day after intervention. Results Limb circumference and pain score in intervention group I and II were significantly shorter and lower than those in control group on 3rd and 7th day after intervention（P=0.043, 0.048, 0.010, 0.030）. Limb swelling overall time（F=3.247, P=0.046）in intervention group I and II was significantly shorter than that in control group. While for adverse reaction incidence, intervention group I was significantly lower than that of intervention group II and control group（χ^2=7.247, P=0.024）. Conclusion In the surrounding treatment of local hoops of body swelling related to snakebite, Jin Huang San modulated with honey has good efficacy and is safer than other agents.
