摘  要:目的探讨子宫内膜厚度与卵巢血液改变的超声诊断在多囊卵巢综合征(PCOS)中的应用。方法彩色多普勒超声检查60例PCOS患者不同时期、促排卵成功、失败时的子宫内膜厚度与卵巢血流动力学改变。结果月经不同时期子宫内膜厚度与卵巢动脉RI比较差异无统计学意义(P>0.05);促排卵成功者卵巢动脉RI在注射HCG当日与黄体中期均显著低于促排卵失败者(P<0.01),促排卵成功患者子宫内膜厚度在注射HCG日与黄体中期均显著大于促排卵失败患者(P<0.05);52例促排卵成功患者中,25例成功妊娠,27例妊娠失败,2组患者卵巢动脉RI比较差异无统计学意义(P>0.05),但妊娠组子宫内膜厚度显著大于未妊娠组(P<0.05)。结论彩色多普勒超声用于PCOS患者不同时期子宫内膜厚度与卵巢血流动力学检查,可为探寻不孕症原因、预测治疗效果提供可靠的非创伤性依据。Objective To explore the clinical value of ultrasound diagnosis of endometrial thickness and ovarian blood changes in treatment of patients with polycystic ovary syndrome (PCOS). Methods Endometrial thickness and ovarian blood changes were detected by ultrasound in 60 patients with PCOS in different periods. Results There were no significant differences in endometrial thickness and ovarian artery RI between different menstrual periods ( P 〉 0.05 ). The RI of ovarian artery at HCG injection day and midluteal phase in successful ovulation patients was significantly lower, and endometrial thickness was significantly higher than unsuccessful ovulation patients (P 〈 0.01 ). In the 52 patients with successful ovulation, 25 patients had successful pregnancy, and 27 cases had unsuccessful pregnancy. There was no significant difference in RI of ovarian artery between the two groups ( P 〉 0.05 ), but endometrial thickness in successful pregnancy patients was significantly higher than unsuccessful pregnancy patients (P 〈 0.05 ). Conclusion Color Doppler ultrasound can provide a reliable basis for the exploration of the causes of infertility in the detection of endometrial thickness and ovarian blood flow dynamics in patients with PCOS in different periods.
