（围）绝经期妇女绝经症状特点及其影响因素分析

摘　　要：目的通过对（围）绝经期专病门诊患者绝经综合征的特点及其影响因素的分析，探讨（围）绝经期综合征评估和治疗方案的标准化管理路径。方法2010年4月至2015年4月就诊于上海交通大学附属第六人民医院（围）绝经期专病门诊患者，采用调查问卷的方法，调查包括人口学特征，采用改良Kupper—nlarl评分表评估绝经症状。结果907例平均年龄（51．74±4．94）（30-70）岁的（围）绝经期妇女纳入研究。（围）绝经期综合征的发生率为95．7％，随绝经时间的延长发生率增加。手术相关的（围）绝经期综合征在发生率和严重程度均高于自然绝经者：发生频率最多的前5位症状依次是：疲乏（84．6％）、失眠（77．3％）、潮热出汗（76．3％）、情绪波动（72．3％）及骨关节痛和心悸（72．1％）；年龄、绝经状态等因素均可影响绝经症状。结论（围）绝经期综合征发生率高．影响因素多．相关手术后的绝经症状在发生率及严重程度均高于自然绝经妇女。Objective To analyze the features of menopausal syndrome and its risk factors in patients from menopausal outpatient clinic, and further to evaluate menopausal syndrome and standardization management pathway of treatment. Methods The objects were enrolled in the menopausal outpatient clinic from Apr. 2010 to May. 2015, completing the self-designed questionnaires through one-by-one interview, including demographic in- formation, and modified Kupperman Index. Results The average age of 907 menopausal women was 51.74±4.94 （aging from 30 to 70 years old） . The incidence of （peri-）menopause syndrome was 95.7%, increasing with the prolonging of menopausal duration. The incidence and severity of surgery-related menopausal syndrome were higher than those of natural menopause. The five most frequent symptoms were： fatigue （84.6%） 〉 insomnia （77.3%） 〉 hot flashes sweating （76.3%） 〉 mood swings （72.3%） 〉 joint pains and heart palpitations （72.1%）. Other factors such as age, menopausal status etc. also affected the menopausal symptoms. Conclusions Prevalence of （peri-） menopausal syndromes was high and it has many influencing factors. The incidence and severity of surgery-relat- ed menopausal syndrome are higher than those of natural menopause.
