摘　　要：目的：回顾性分析39例尺骨撞击综合征患者的MRI表现，以提高对尺骨撞击综合征患者腕部损伤的影像认知，降低患者月骨和/或三角骨坏死的发生。方法采用Gelberman等的平行线法，对39例尺骨撞击综合征患者腕部MRI图像进行测量及分析。结果尺骨阳性变异29例（74.4%），变异范围为2～4.26 mm，尺骨阴性变异8例（20.5%），变异范围为0.75～1.77 mm，尺骨中性变异2例。其中月骨损伤28例，三角骨损伤11例，三角纤维软骨盘损伤38例。MRI显示损伤主要位于月骨尺侧部和三角骨腰部，且除1例之外，其余患者均伴有三角纤维软骨的损伤。结论 MRI可早期发现尺骨撞击综合征月骨、三角骨损伤的改变，并且对于仅有三角纤维软骨盘损伤者，如果伴有临床症状或相应的临床病史，要高度怀疑尺骨撞击综合征，以提高该病的诊断率及降低月骨和/或三角骨坏死的发生率。ObjectiveA retrospective analysis of MRI findings in 39 cases of ulnar impaction syndrome, to improve the image perception in ulnar impaction syndrome wrist injury, reduce patients with lunate and/or triangular bone necrosis.Methods By using Gelberman parallel line method, MRI ifnding analysis is done in 39 cases of ulnar impaction syndrome patients.Results Of 29 cases of ulnar positive variance （approximately 74.4%）, the positive variance is in the range of 2~4.26 mm; Of 8 cases of ulnar negative variance （approximately 20.5%）, the negative variance is in the range of 0.75~1.77 mm and 2 cases of ulnar neutral variance. As in 28 cases there were lunate bone injury, 11 cases of triquetrum bone injury, triangular ifbrocartilage complex （TFCC） disc injury in 38 cases. MRI shows chondromalacia of the lunate was presented on the side of ulnar and the triquetrum bone and almost all the patients with damage to the TFCC.Conclusion MRI can suggest early detection of ulnar impaction syndrome, lunate injury, triquetrum bone injury and for only the TFCC disc injury, if accompanied by clinical symptoms or appropriate clinical history then it should be highly suspected of ulnar impaction syndrome, in order to reduce lunate bone and/or triquetrum incidence of necrosis.
