可弯曲喉罩在小儿麦粒肿手术中的应用

摘　　要：目的比较可弯曲喉罩(FLMA)与加强型气管导管(ET)在小儿麦粒肿手术中的应用效果。方法选择择期全麻下麦粒肿切除术患儿40例,随机均分为FLMA组(F组)和ET组(E组)。记录麻醉诱导即刻(T1)、诱导后2.5 min(T2)、插管后1 min(T3)、手术结束即刻(T4)、拔管后1 min(T5)各时点心率,血压,术中最高吸气峰压,呼吸回路到术野中心的距离以及置管失败、术中漏气、套囊染血、拔管咳嗽的发生率。结果 F组在T3及T5时刻心率、血压低于E组(P<0.05)。F组比E组术中气道吸气峰压更低(P<0.001),呼吸回路距术野中心更远(P<0.001)。2组在置管失败等事件的发生率上无统计学差异(P>0.05)。结论FLMA可以安全应用于小儿麦粒肿手术中,其通气效果良好、血流动力学稳定、对手术术野干扰小。Objective To compare the chnical efficacy of the tlexible laryngeal mask airway （FLMA） with the reinforced endotracheal tube （ET） in the pediatric hordeolum surgery. Methods Forty hospitalized children scheduled hordeolum surgery under general anesthesia, were as- signed into two groups randomly： group F （FLMA group） and group E （ET group）.The value of the heart rate （HR）, systolic blood pressure （SBP）, mean arterial pressure （MAP）and diastolic blood pressure （DBP）in the two groups were recorded before the anesthesia induction （T1）, at 2.5 rain after anesthesia induction （T2）, at 1 min after intubation （T3）, at the end of surgery （T4） and at 1 min after extubation （T5）. The peak air- way pressure, the distance from breathing circuit to surgery field center and the occurring rate of adverse events such as failed intubation, airway leaking during surgery, blood contaminating of cuff and persisted coughing after extubation were also recorded. Results Compared with group E, the value of HR, SBP, MAP and DBP in group F were lower at T3 and T5 （P 〈 0.05 ）. Group F showed lower peak airway pressure （P 〈 0.001 ） and further distance from breathing circuit to surgery field center compared with group E （P 〈 0.001 ）. There was no difference between two groups in the occurring rate of adverse events, such as failed intubation （P 〉 0.05）. Condusion The apphcation of FLMA in pediatric hordeolum sur- gery can keep the airway safety and has good ventilation effect, stable hemedynamics and less influence to surgery.
