背部弹力纤维瘤的 MSCT 诊断

摘　　要：目的：分析背部弹力纤维瘤（EFD）的 MSCT 表现。方法收集22例经手术病理证实并行胸部 CT 检查的 EFD 患者资料,分析其临床特点及 CT 表现特征。结果22例患者中 CT 共发现38处病变,并均位于背部肩胛下角区,前锯肌和背阔肌深层。16例（72.7%）病灶累及双侧,5例病变仅位于右侧,1例仅位于左侧。右侧病变直径略大于左侧（6.2 cm±1.3 cm vs 5.7 cm±1.6 cm）。CT 平扫显示病变位于肩胛下角肌层深部,呈扁丘状或半圆形,以肌样密度为主,病灶内见脂肪密度沿病变长轴条纹状排列（19例）和不规则岛状分布（3例）；病变边界不规整,周围脂肪间隙模糊。9例患者行增强检查,3例病灶轻度强化,6例无强化。结论CT发现中老年女性单侧或双侧肩胛下区无症状扁丘状肿块、呈肌样密度并条纹状脂肪间隔排列、无或轻度强化者应考虑 EFD。Objective To analyze the MSCT findings of elastofibroma dorsi（EFD）.Methods 22 patients with EFD confirmed by surgeryand pathology were involved.All patients underwent MSCT.The Clinical features and CT characteristics of EFD were analyzed retrospectively. Results Totally 22 patients with 38 lesions found with MSCT.All 38 lesions were located in the inferior section of the scapula,deep within the anterior serratus and latissimus dorsi musculature.Bilateral lesions were found in 1 6 patients（72.7%）.Lesions were only on the right side in 5 patients（22.7%）,and on the left side in 1 patient （4.5%）.The mean diameter of lesions on the right side was a little bigger than that on the left side（6.2 cm±1.3 cm vs 5.7 cm±1.6 cm）.The features of MSCT were all located in the deep subscapular muscle in the back,mainly with flat mound-like shape or semi-circular shape,isodensity like muscle,interlacing of linear streaks of fat densities in 1 9 patients and distributing as irregular islands in 3 patients.The masses were irregular edged,and fat gaps were turbid.Of 9 patients who underwent contrast enhanced CT,the lesions slightly enhanced in 3 patients and had no enhancement in 6 patients.Conclusion EFD should be considered when asymptomatic masses in unilateral or bilateral subscapular areas show muscle-like density with striated fat space,with no or slightly enhancing on CT in middle-aged women.
