前哨淋巴结活检术在甲状腺癌手术中的应用

摘　　要：目的探讨前哨淋巴结活检(SLNB)在甲状腺癌手术中的应用价值。方法采用亚甲蓝染色法对57例可疑甲状腺癌患者行术中SLNB,前哨淋巴结和颈淋巴结清扫术切除的淋巴结送常规HE染色病理检查,并将两者进行对照。结果前哨淋巴结检出率为91.23%,灵敏度、准确度、阳性预测值以及阴性预测值分别为92.00%、87.72%、100%和93.10%。Ⅳ区SLN阳性率较高(P<0.05)且与肿瘤大小有一定相关(P<0.05)。结论甲状腺癌SLNB具有可行性,对于甲状腺癌术中淋巴结清扫具有一定的指导意义。Objectives To explore the application value of sentinel lymph node biopsy（ SLNB） for thyroid cancer surgery,the study was conducted.Methods SLNB was performed in 57 patients with suspected thyroid cancer by methylene blue staining.The sentinel lymph nodes and lymph nodes that were resected by lymph node dissection were sent for pathologic examination.Two kinds of results were compared.Results The detection rate of sentinel lymph nodes was 91.23%.The sensitivity,accuracy,positive predictive value and negative predictive value were 92%,87.72%,100% and 93.10% respectively.The positive rate of SLN was higher in IV district（ P〈 0.05） and there was a certain correlation with tumor size（ P〈 0.05）.Conclusion The SLNB for thyroid carcinoma is feasible and it may provide guidance for extent of neck lymph node dissection.
