甘肃省中医医疗机构输血科建设与管理现状调查

摘　　要：目的:了解甘肃省中医医疗机构输血科(血库)现状及存在的问题,以督促其持续改进,逐步达到国家卫计委《医疗机构临床用血管理办法》要求。方法:通过查资料和现场检查,对甘肃省18家中医医疗机构设置和人力资源、房屋面积和仪器设备、临床用血管理等方面进行调研。结果:18家中医医院均成立了输血管理委员会,有4家医院独立设科,其余均隶属于检验科,共有专业技术人员45人,其中高级职称占8.88%(4/45);中级职称22.2%(10/45);初级职称66.7%(30/45),18个输血科(血库)房屋面积达标5家,布局符合卫生学要求4家;科基础设备均不够齐全;临床管理和合理输血合格率60.0%(27/45)。结论:甘肃省中医医疗机构输血科(血库)建设和临床输血管理逐步走向正规,但房屋建设薄弱,人员以初级职称为主,输血新技术新业务,教学及科研能力薄弱,临床输血有效性评估及各规章制度有待加强,特别是输血管理软件,LIS和HIS信息系统在县级中医院普及率较低。Objective： To promote the continuous improvement of blood transfusion department in Gansu Provincial TCM medical institution, therefore to meet the requirements of clinical blood management of medical in- stitutions of National Health Planning Commission gradually by surveying the status of blood transfusion department of Gansu provincial TCM medical institution. Methods： Structural establishment, human resources, floor space, e- quipment and clinical blood management of 18 TCM medical institutions were investigated by checking the data and field inspection. Results： Blood transfusion management committee had been established in 18 TCM hospitals, there were four hospitals establishing the departments independently, other hospitals established the committee belonging to clinical lab, and there were 45 professionals, senior professional title 8.88%（4/45）, medium grade professional title 22.2%（10/45）; and primary professional title 66.7%（30/45）, there were five departments with floor space meeting the requirements among 18 blood transfusion departments, and four departments with the layout meeting the require- ments of the hygiene; and inadequate basic equipments of blood transfusion department, only one equipped with platelet constant temperature shock box, improper storage would lead to blood platelet infusion failure; clinical man- agement and rational blood transfusion pass percentage reached 60.0%（27/45）. Conclusion： The construction of blood transfusion department in Gansu provincial TCM medical institution and the management of clinical blood transfu- sion begin to be standard gradually, but house construction is weak, the persons are mainly with primary professional titles, new technology and new task of blood transfusion, teaching and scientific innovation ability are weak, clinical blood transfusion effectiveness evaluation and all the regulations need to be enhanced, especially blood transfusion management software, and the popularity rate of LIS and HIS information system is low i
