摘 要:目的观察新生儿凝固酶阴性葡萄球菌(CNS)败血症患儿血降钙素原(PCT)、C反应蛋白(CRP)、白细胞(WBC)及血小板(PLT)水平的变化,探讨其与新生儿CNS败血症的关系。方法选择2013年至2014年住院的新生儿CNS败血症患儿38例作为观察组,另选同时期收住无感染症状的32例新生儿为对照组,比较两组患儿入院后血PCT、CRP、WBC及PLT水平和阳性率的差异,并观察这些指标诊断新生儿CNS败血症灵敏度和特异度。结果观察组PCT水平、阳性率显著高于对照组(P<0.05),两组患儿CRP、WBC、PLT水平及阳性率比较差异无统计学意义(P>0.05),PCT诊断新生儿CNS败血症灵敏度高于CRP、WBC及PLT,但特异度低于后三者。结论新生儿CNS败血症无特异性实验室检查方法,PCT对新生儿CNS败血症早期诊断有一定的价值,联合检测PCT、CRP、WBC及PLT能提高诊断的准确率,血培养阳性仍是诊断新生儿CNS败血症的金标准。Objective To observe the changes of blood procalcitonin (PCT), C-reactive protein (CRP), white blood cell (WBC) and platelet (PLT) and their association with septicemia caused by coagulase-negative Staphylococci (CNS) in neonates. Methods 38 neonates with septicemia caused by CNS admitted in our hospital from 2013 to 2014 were selected as the observation group, and 32 neonates without symptoms of infection in the same period were included as the control group. The levels and positive rate of blood PCT, CRP, WBC and PLT of the neonates in the two groups were compared after admission. The sensitivity and specificity of each index for the diagnosis of neonatal septicemia caused by CNS were observed. Results The levels and positive rate of serum PCT of the newborns in the observation group were significantly higher than in the control group (P〈0.05). There was no statistical significance in the levels and positive rates of CRP, WBC and PLT of the neonates between the two groups (P〉0.05). The sensitivity of PCT for the diagnosis of neonatal septicemia caused by CNS was higher than CRP, WBC and PLT, but the specificity of PCT was lower than CRP, WBC and PLT. Conclusion There is no specific laboratory examination for neonatal septicemia caused by CNS. The level of PCT is a valuable predictor for early-stage neonatal septicemia caused by CNS, which can help improve the diagnostic accuracy when combined with the levels of CRP, WBC and PLT. The positive blood culture remains the gold standard for the diagnosis of neonatal septicemia caused by CNS
