聚焦解决模式在腰椎间盘突出症术后患者家庭随访管理中的应用

摘　　要：目的:探讨聚焦解决模式在腰椎间盘突出症术后患者家庭随访管理中的应用效果。方法:将84例首次行腰椎间盘切除术患者随机等分为观察组和对照组,观察组患者采取聚焦解决模式进行随访管理;对照组患者采取常规随访管理,随访持续6个月,比较两组患者在随访前后自我护理能力(ESCA)、腰椎JOA功能评分、生存质量(SF-36)及术后复发率的情况。结果:观察组干预后ESCA评分、SF-36各维度评分均高于对照组(P<0.05);观察组在术后1,3,6个月时JOA评分均高于对照组;观察组术后6个月的复发率低于对照组(P<0.05)。结论:基于聚焦解决模式的随访管理可有效提升腰椎间盘突出症术后患者自我护理能力,改善腰椎功能状态及生存质量,降低术后腰椎间盘突出症的复发率。Objective ： To explore the application effects of solution - focused approach in home follow - up management of lumbar disc herniation patients af- ter operation. Methods ： Randomly divided 84 patients taken lumbar discectomy for the first time into observation group and control group, patients in the ob- servation group conducted follow - up management with solution - focusd approach ; patients in control group conducted routine follow - up management, the follow - up lasted for 6 months, compared the exercise of self - care ability （ ESCA）, lumbar JOA function scores, quality of life （ SF - 36） and postopera- tive recurrence rate of patients in the two groups before and after the follow - up. Results ： After the intervention, the observation＇ s ESCA scores, various di- mension scores of SF - 36 were all higher than those of the control group （ P 〈 0.05 ） ; at the 1~t , 3rd and 6th month after the operation, the observation group＇ s JOA scores were all higher than those of the control group; the observation group＇ s recurrence rate of the 6 months after the operation was lower than that of the control group （ P 〈 0.05 ）. Conclusion ： The follow - up management based on solution - focused approach can effectively enhance the self - care ability of lumbar disc herniation patients after the operation, improve lumbar function status and quality of life and reduce the postoperative recurrence rate of lumbar disc herniation.
