闭环管理模式在护理隐患排查处理中的应用

摘　　要：目的：探讨闭环管理模式在护理隐患排查处理中的应用及效果。方法：分析2013年1-12月我院呼吸科病房重症患者的资料，汇总护理不良事件发生情况、发生的原因，建立闭环管理小组，从重症患者病情观察、护理风险预警、应急处理流程、护理管理制度、信息沟通等方面形成关于重症患者护理隐患排查的闭环管理模式，使该模式能产生强大的自激力，达到提高护理质量、降低护理隐患的良性循环效果。结果：2013年1-12月共发生护理不良事件53例，导致护理隐患发生的原因包括对患者病情观察不仔细、风险预知不谨慎、信息沟通不畅、护理操作不娴熟、应急处理不及时不规范等。后期我院开始应用闭环管理模式以加强护理管理，2014年1～12月护理不良事件发生11例，与闭环管理模式应用前相比有明显降低，且管理模式实施后患者对护理工作的满意度明显高于管理模式实施前，差异具有统计学意义（P〈O．05），受表扬护士人次也明显提高。结论：闭环管理模式应用于护理隐患排查处理中的效果确切，可明显降低护理不良事件发生率，提高患者满意度。Objective：To explore the application and effects of closed - loop management mode in hidden nursing danger troubleshooting and handling. Methods：Analyzed the information of critically ill patients in respiratory department of our hospital from January to December in 2013, summarized the con- dition and causes of nursing adverse event occurrence, established closed - loop management team, formed a closed - loop management mode about critical- ly ill patients＇ hidden nursing danger troubleshooting and handling from aspects of critically ill patients＇ medical condition observation, early warning of nursing risk, emergency handling process, nursing management system and information communication, making the mode generate powerful self - exited force to achieve virtuous circle effects of enhancing nursing quality and reducing hidden nursing danger. Results ：There were totally 53 cases of nursing ad- verse events from January to December in 2013, reasons causing hidden nursing dangers included careless observation on patients＂ medical condition, un- scrupulous risk prediction, unsmooth information communication, non - skillful nursing operation, unresponsive and nonstandard emergency treatment, etc. In later period, our hospital began to adopt closed -loop management mode to strengthen nursing management, from January to December in 2014, there were 11 cases of nursing adverse event, significantly lower than the incidence before the application of closed -loop management mode, patients＇ satisfac- tion on nursing work after the application of the mode was significantly higher than the one before the application of the management mode, the differences were of statistical significance （P 〈 0.05 ）, and the number of people being praised was significantly improved as well. Conclusion： Applying closed - loop management mode to hidden nursing danger troubleshooting and handling has exact effects in significantly lowering the incidence of nursing adverse event and improving patients＇ satisfaction.
