内镜逆行阑尾炎治疗术应用多中心回顾性分析

摘　　要：目的分析我国多个中心进行内镜逆行阑尾炎治疗术（endoscopicretrogradeappendicitistherapy，ERAT）的应用现状，探讨ERAT的有效性和安全性。方法收集2009—2014年间8所医院接受ERAT诊治并符合一定条件的患者资料。总结ERAT阑尾插管成功率、内镜治疗成功率、住院时间、并发症、复发情况等。结果纳入分析的118例患者中，107例插管成功，插管成功率为91％。7例患者排除急性阑尾炎，避免阑尾阴性切除。100例患者确诊为急性阑尾炎并给予内镜治疗，97例自觉腹痛缓解时间平均为12h（6～72h），腹部压痛消失时间平均为24h（24～72h），内镜治疗成功率为97％。患者住院时间平均3d（2—4d）。3例患者治疗失败，其中2例在术中发现穿孔归为并发症，并发症发生率2％。平均随访12个月，复发7例（7％）。结论ERAT能够在明确诊断的基础上，高效、安全、微创地治疗急性阑尾炎。Objective To evaluate the safety and efficacy of endoscopic retrograde appendicitis therapy （ERAT） by analyzing application of the technique at different centers in China. Methods Data of patients who underwent ERAT at 8 tertiary hospitals in China from 2009 to 2014 were retrospectively studied for the success rate of cannulation, clinical success rate, hospital stay, complications and recurrent rate. Re- suits There were 107 in 118 patients （ 91% ） whose appendiceal orifice was successfully cannulated. Acute appendicitis（AA） was excluded in 7 patients. One hundred （including 6 patients with peri-appendiceal ab- scess） had definite diagnosis of AA and received ERAT. The mean time of abdominal pain and abdominal tenderness relief was 12 hours （6-72 hours） and 24 hours （24-72 hours） respectively. The clinical success rate was 97%. The mean time of hospital stay was 3 days （2-4 days）. The rate of complications was 2%. During the mean time of follow-up （ 12 months）, the recurrent rate was 7%. Conclusion ERAT is safe and effective for the diagnosis and treatment of acute appendicitis and peri-appendiceal abscess.
