红花酒精湿敷治疗自体动-静脉内瘘并发症的疗效

摘　　要：目的探讨红花酒精湿敷对自体动-静脉内瘘并发症的疗效和安全性。方法自体动-静脉瘘发生皮下血肿血液透析患者60例随机均分为两组,在常规热敷的基础上分别兼用红花酒精湿敷(A组)和喜辽妥外搽(B组)处理,连续治疗4周,比较两组疗效。结果治疗后,两组局部血肿、疼痛和血流量情况均有改善。A组血肿消退有效率和疼痛缓解有效率均高于B组(96.7%vs.83.3%和96.7%vs.86.3%)(P<0.05);治疗后1、6个月,A组内瘘血流量也大于B组(P<0.05)。结论在常规热敷的基础上,红花酒精湿敷较喜辽妥外搽更能有效促进血液透析患者自体动-静脉内瘘并发皮下血肿的吸收,改善血流量,缓解局部疼痛,预防和延缓内瘘狭窄及栓塞形成。Objective To observe the efficacy and safety of Safflower alcohol preparation wet compress on the complications of internal arteriovenous fistula. Methods On the basis of conventional hot compress, 60 patients undergoing hemodialysis with the complications of internal arteriovenous fistula were additionally treated with Safflower alcohol wet compress（group A, 30 cases） or hirudoid ointment（group B, 30 cases）. The efficacy was compared after treated for 4 weeks. Results After treatment, local hematoma, pain and blood flow of two groups were all improved. The effectiveness rates of hematoma fade and pain relief were significantly higher in group A than those in group B （96.7% vs. 83.3% and 96.7% vs. 86.3%）（P〈0. 05）. The blood flow of internal arteriovenous fistula was greater in group A than that in group B in one and 6 months after treatment（p〈0. 05）. Conclusion On the basis of conventional hot compress, additional use of Safflower alcohol wet compress produces better efficacy than birudoid ointment applying in the treatment of complications of internal arteriovenous fistula in the patients undergoing hemodialysis.
