解毒化瘀方加减对乙型病毒性肝炎伴高胆红素血症肝纤维化及凝血功能的影响

摘　　要：目的:观察解毒化瘀方加减对乙型病毒性肝炎伴高胆红素血症患者肝纤维化及凝血功能的影响。方法:选择2014年6月至2015年6月我院80例乙型病毒性肝炎伴高胆红素血症患者作为研究对象,随机分为两组,每组40例。对照组单纯行西医常规治疗,观察组在常规治疗基础上加用解毒化瘀方加减治疗。观察两组治疗效果及治疗后对肝纤维化及凝血功能的影响。结果:观察组治疗总有效率为90%,显著高于对照组(72.5%),差异有统计学意义(P<0.05)。观察组治疗后层粘连蛋白(LN)、透明质酸(HA)、Ⅲ型前胶原(PCⅢ)及Ⅳ型胶原(Ⅳ-C)显著低于治疗前和对照组治疗后,差异有统计学意义(P<0.05)。观察组治疗后活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白原(FIB)及凝血酶时间(TT)与治疗前和对照组治疗后比较,差异有统计学意义(P<0.05)。结论:解毒化瘀方加减治疗病毒性乙肝合并高胆红素血症疗效显著,可有效控制肝纤维化进程,改善凝血功能。Objective： To study the effects of Modified Jiedu Huayu Decoction on hepatic fibrosis and coagulation function in patients with viral hepatitis B complicated with hyperbilirubinemia. Methods： 80 patients with viral hepatitis B complicated with hyperbilirubinemia who were admitted in our hospital between June 2014 and June 2015 were selected as the study objects and were randomly divided into two groups,40 cases in each group. Based on routine therapy,the observation group was treated with Modified Jiedu Huayu Decoction. The controlled group was only treated with western medicine. The curative effects and effects on hepatic fibrosis and blood coagulation function in the two groups after treatment were observed. Results： The total effective rate in the observation group（ 90%） was significantly higher than that in the controlled group（ 72. 5%）（ P 0. 05）. After treatment,laminin（ LN）,hyaluronic acid（ HA）,type III procollagen（ PC III） and type IV collagen（ IV-C） in the observation group were significantly lower than those before treatment and those in the controlled groups after treatment（ P0. 05）. After treatment,the activated partial thromboplastin time（ APTT）,prothrombin time（ PT）,fibrinogen（ FIB） and thrombin time（ TT） of the observation group were respectively. Compared with those before treatment and those of the controlled group after treatment,there were significant differences（ P0. 05）. Conclusion： The curative effect of Modified Jiedu Huayu Decoction in viral hepatitis B complicated with hyperbilirubinemia is significant. It can effectively control the progression of hepatic fibrosis and improve blood coagulation function.
