妊娠期肝内胆汁淤积症与新生儿出生体重的关系

摘　　要：目的探讨妊娠期肝内胆汁淤积症(ICP)与新生儿出生体重的关系及临床意义。方法选取2012年5月-2015年5月在该院住院的150例ICP患者为研究对象,根据患者的诊断孕周分为两组,其中32周前确诊的患者为早发型组(A组),32周后确诊的患者为晚发型组(B组)。回顾性对比分析两组患者年龄,诊断孕周、分娩孕周,诊断至分娩间期,新生儿体重、肝酶和凝血指标的差异。结果两组诊断至分娩间期以及新生儿出生体重比较,差异有统计学意义(P<0.05),而两组年龄、分娩孕周、肝酶和凝血指标比较,差异无统计学意义(P>0.05)。相同的分娩孕周,早发型ICP孕妇分娩新生儿的体重低于晚发型ICP孕妇分娩的新生儿。结论 32周前确诊的早发型ICP与新生儿出生低体重相关。Objective To explore the relationship between intrahepatic cholestasis of pregnancy （ICP） and neonatal birth weight and the clinical significance. Methods A total of 150 patients with ICP were selected from the hospital from May 2012 to May 2015, then the patients were divided into two groups according to the gestational week at the time of diagnosis： the patients in group A （early-onset group） were diagnosed definitely before 32 gestational week, while the patients in group B （ late-onset group） were diagnosed definitely after 32 ges- tational week. Age, gestational week at the time of diagnosis, gestational weeks at birth, the interval time between diagnosis and delivery, neonatal birth weights, liver enzymes, and coagulation parameters in the two groups were compared retrospectively. Results There were sta- tistically significant differences in the interval time between diagnosis and delivery and neonatal birth weight between the two groups （P〈 0. 05 ） . There was no statistically significant difference in age, gestational week, liver enzymes, and coagulation parameters between the two groups （ P〉0. 05 ） . In the same gestational week at birth, birth weight of neonates born by pregnant women with early-onset ICP was lower than that of neonates born by pregnant women with late-onset ICP. Conclusion Early-onset ICP diagnosed definitely before 32 gestational weeks is correlated with neonatal low birth weight.
