宫颈上皮内瘤变LEEP术后阴道局部用药疗效分析

摘　　要：目的探讨不同药物在宫颈上皮内瘤变(Cervical Intraepithial Neoplasia,CIN)环形电切除术(Loop Electrosurgical Excision Procedure,LEEP)术后的疗效。方法将113例CIN患者随机分为辛复宁(重组人干扰素α-2b阴道泡腾胶囊)组、爱杰特(纳米银妇女外用抗菌凝胶)组和对照组,均行LEEP术,术后给予口服抗生素48 h预防感染。辛复宁组和爱杰特组分别给予辛复宁、爱杰特阴道后穹窿上药3个疗程,对照组常规处理,观察3组患者术后创面愈合时间、伴随症状持续时间、术后并发症、人乳头瘤状病毒转阴率。结果辛复宁组、爱杰特组和对照组在愈合时间、伴随症状持续时间、并发症发生率、HPV转阴率等方面存在不同程度差异,其中辛复宁在以上各方面均存在优势。结论 CIN术后阴道局部用药治疗对术后恢复有积极意义;辛复宁明显缩短了创面愈合时间并减少了并发症的发生,提高了HPV转阴率,值得临床推广。Objective To explore the curative effects of different drugs after loop electrosurgical excision procedure （LEEP） for cervi- cal intraepithelial neoplasia （CIN） . Methods A total of 113 patients with CIN were randomly divided into Xinfuning group （recombinant human interferon α-2b vainal effervescent capsule）, Aijiete group （nano-silver external antimicrobial gel for women）, and control group. All the patients were treated by LEEP and oral antibiotics for 48 hours to prevent postoperative infection. The patients in Xinfuning group and Aijiete group were treated by vaginal fornix irrigation of Xinfuning and Aijiete for three courses of treatment respectively ; the patients in control group were treated by conventional therapy. The healing time of wounds after LEEP, duration time of simultaneous phenomenons, postoperative complications, and HPV negative conversion rates in the three groups were observed. Results There were differences in healing time of wounds after LEEP, duration time of simultaneous phenomenons, the incidence rate of complications, and HPV negative conversion rate among the three groups in varying degrees, Xinfuning in the above-mentioned areas had superiorities. Conclusion Vaginal therapy has positive effect on wound healing in treatmen of CIN. Xinfuning obviously shortens wound healing time, reduces the incidence rate of complications, and improves HPV negative conversion rate, which is worthy of clinical promotion.
