输尿管软镜碎石术与经皮肾镜碎石术治疗鹿角形肾结石疗效比较

摘　　要：目的比较输尿管软镜碎石术(FURS)与经皮肾镜碎石术(PNL)治疗鹿角形肾结石的临床疗效。方法选择2014年6月至2016年1月新乡市中心医院收治的42例鹿角形肾结石患者,根据患者意愿分为FURS组(n=22)和PNL组(n=20),对2组患者手术并发症发生率、手术时间、结石清除率、术后住院时间等进行比较。结果 FURS组和PNL组患者手术并发症总发生率分别为18.2%(4/22)和10.0%(2/20),手术时间分别为(127.7±55.9)和(97.9±50.8)min,结石清除率分别为68.2%(15/22)和95.0%(19/20),术后住院时间分别为(5.5±1.1)和(7.0±1.3)d,2组患者手术时间、结石清除率及术后住院时间比较差异均有统计学意义(P<0.05)。结论 FURS和PNL均是鹿角形肾结石有效的治疗方法,PNL有更高的结石清除率、更短的手术时间,而FURS创伤更小、恢复更快、住院时间更短。Objective To investigate the clinical curative effect of flexible ureteroscopy （ FURS ） and percutaneous nephrolithotomy（PNL） in the treatment of renal staghom calculi. Methods Forty-two patients with renal staghom calculi in the Xinxiang Central Hospital from June 2014 to January 2016 were selected and divided into FURS group（ n = 22） and PNL group（n =20） according to the patients＇ wish. The incidence of surgical complications,operation time,stone-free rate,postop- erative hospital stay were analyzed and compared between the two groups. Results The incidence of surgical complication, op- eration time, stone-free rate, postoperative hospital stay of patients in FURS group were 18.2% （4/22）, （127.7 ± 55.9 ）rain, 68.2% （15/22） and （5.5 ± 1.1 ）d respectively;the operation time, stone-free rate and postoperative hospital stay of patients in PNL group were 10.0% （2/20） , （ 97.9 ± 50.8） min,95.0% （ 19/20）, （7.0 ±1.3 ） d; there were statistic difference of above indexes of patients between the two groups （ P 〈 0. 05 ）. Conclusions Both FURS and PNL are effective treatment methods for renal staghorn calculi. PNL provides higher stone-free rate and has shorter operation time;FURS provides minimal invasion, fas- ter recovery and short hospitalization time.
