中医外治联合骶管滴注治疗腰椎间盘突出症临床疗效观察

摘　　要：目的:观察中医外治联合骶管滴注治疗腰椎间盘突出症的临床疗效。方法:将60例腰椎间盘突出症患者采用随机抽样法分为中医外治+骶管滴注组和单纯骶管滴注组,每组30例。所有患者接受每周1次、持续3周的治疗,并在指导下进行腰背肌锻炼。评定治疗前及治疗后不同时期腰椎JOA评分及腰痛视觉模拟评分(VAS),治疗结束后评定治疗总体有效率,并进行组间比较。结果:中医外治+骶管滴注组腰痛VAS低于单纯骶管滴注组,在治疗1周后差异具有统计学意义(P<0.05)。治疗结束后中医外治+骶管滴注组腰椎JOA评分也好于单纯骶管滴注组(P<0.05)。治疗总有效率中医外治+骶管滴注组高于单纯骶管滴注组(P<0.05)。结论:中医外治法能减轻腰椎间盘突出症患者的疼痛,改善其功能,与骶管滴注疗法合用可提高临床治疗效果。Objective： To observe the curative effect external therapy of triditional Chinese medicine（ TCM）combined sacral canal injection on prolapse of lumbar intervertebral disc. Methods： Sixty patients with prolapse of lumbar intervertebral disc were randomly divided into external therapy of TCM combined sacral canal injection group and only sacral canal injection group,30 in each group. All patients received treatment once a week and lasted 3 weeks and waist back muscle practice was guided. Lumbar JOA,visual analog scale（ VAS） of lumbago before treatment and at the end of each treatment cycle were evaluated. The overall treatment efficiency were assessed and compared between the two groups. Results： VAS of lumbago in external therapy of TCM combined sacral canal injection group was less than that in only sacral canal injection group. The difference was statistically significant after one week treatment（ P〈0. 05）. The JOA scores of patients in external therapy of TCM combined sacral canal injection group were significantly better than those in only sacral canal injection group in the end of treatment（ P〈0. 05）. Effective rate of external therapy of TCM combined sacral canal injection group was significantly better than that in only sacral canal injection group（ P〈0. 05）. Conclusions： External therapy of TCM can reduce the pain of patients with prolapse of lumbar intervertebral disc. It can improve the function of patients. It has a better clinical therapeutic effect combines with therapy of sacral canal injection.
