摘 要:目的评价黄芪颗粒对稳定期慢性阻塞性肺病(COPD)患者的临床疗效以及对诱导痰中超氧化物歧化酶(SOD)、丙二醛(MDA)和白介素(IL)-17的影响。方法 40例重度或极重度稳定期COPD患者随机分成对照组和治疗组,每组各20例。对照组患者使用信必可都保联合噻托溴铵吸入,治疗组患者在此基础上联合黄芪颗粒口服,治疗24周后分别观察肺功能、COPD评估测试(CAT)评分、诱导痰中炎性细胞计数及分类,SOD、MDA和IL-17的水平变化。结果 2组患者治疗后1秒钟用力呼气容积(FEV1)和FEV1占预计值百分比(FEVl%)以及CAT评分均显著改善(P均<0.01),且治疗组疗效优于对照组(P均<0.05);2组患者治疗后诱导痰中的炎性细胞总数和中性粒细胞比例均下降(P<0.01),且治疗组疗效优于对照组(P<0.05);2组患者治疗后诱导痰中IL-17和MDA水平均下降、SOD水平升高(P均<0.01),且治疗组疗效优于对照组(P均<0.05)。结论黄芪颗粒可改善稳定期COPD患者肺功能,提高患者生活质量,抑制气道局部氧化应激和气道炎症。Objective To observe the clinical effects and influence of astraglus pellets on the levels of SOD, MDA and IL-17 in induced sputum of patients with stable chronic obstructive pulmonary disease (COPD). Methods Forty patients with severe and very severe COPD were randomly divided into control group and treatment group with 20 patients in each group. The control group was treated with symbicortturbuhaler and tiotropium bromide. The treatment group was treated with astraglus pellets on the basis of control group. 24 weeks after treatment,the pulmonary function, CAT scores, the total number of inflammatory cells or classification, and the levels of interleukin( IL)-17, SOD and MDA in induced sputum were detected and compared in the two groups. Results The forced ecpiratory volume in the first second(FEV1), FEV1 predicted (FEVI%) and CAT scores were improved significantly after treatment in two groups (all P〈0.01 ), and they were better in treatment group than those in control group (all P〈 0. 05). The total number of inflammatory cells and percentage of neutrophils was decreased in induced sputum after treatment in both groups ( all P〈 0.05 ). The treatment group was superior to the control group(all P〈0. 05); After treatment, the levels of IL-17 and MDA in induced sputum were reduced, the level of SOD in induced sputum was increased in both groups ( all P〈 0.01 ). The curative effect of treatment group was better in treatment group than that in control group ( all P〈0. 05). Conclusions Astraglus pellets can improve pulmonary function, inhibit airway local oxidative stress and airway inflammation in stable COPD patients.
