双胎之一胎死宫内孕妇围产期哀伤量表的编制及信效度研究

摘　　要：目的：编制双胎之一胎死宫内（ sIUFD）孕妇围产期哀伤量表,并评价其信度和效度,为此类孕妇的心理护理提供相应的参考依据。方法成立量表编制研究小组,以围产期哀伤量表（ PGS）为框架,通过头脑风暴法、专家函询法以及预试验最后形成24个条目的sIUFD孕妇围产期哀伤量表,并对2013年1月—2015年12月100例sIUFD不同孕周的孕妇进行临床应用评价,评价量表的内部一致性信度、重测信度、内容效度,采用受试者工作特征曲线（ ROC）分析量表的灵敏度、特异度等。结果 sIUFD孕妇围产期哀伤量表的Cronbach′s α系数为0.896,分半信度为0.806,重测信度为0.901（P〈0.01）；内容效度指数为0.92。除第15条,sIUFD 孕妇围产期哀伤量表其他条目与总分的相关系数较好（ r =-0.263-0.779）,以PGS校标,sIUFD孕妇围产期哀伤量表与PGS具有显著相关性（r=0.845,P〈0.001）。 sIUFD孕妇围产期哀伤量表的灵敏度为0.71,特异度为0.72,分界值为72分。结论 sIUFD孕妇围产期哀伤量表具有良好的信效度和敏感度,对评定sIUFD孕妇的哀伤情绪具有重要临床应用价值。Objective To design perinatal grief scale （ PGS） for single intrauterine fetal death（ sIUFD） and evaluate its reliability and validity, so as to provide reference for psychological nursing of this kind of pregnant women. Methods Scale development study group was established, with the PGS as a framework, 24 items were enrolled after brainstorming, expert consultation and preliminary experiments. From January 2013 to December 2015, 100 pregnant women with sIUFD were enrolled from this hospital. Internal consistency reliability, retest reliability, content validity, and receiver operating characteristic curve （ ROC） were used to analyze the sensitivity, specificity, etc. Results Cronbach′s α coefficient and split-half reliability of the scale were 0. 896 and 0. 806. Test-retest reliability and content validity index were 0. 901 and 0. 92. Except for the fifteenth item,the other items had a good correlation with total score （r = -0. 263 -0. 779）. With PGS as calibration, PGS of sIUFD had a good correlation with PGS （r =0. 845, P 〈0. 001）. The sensitivity and specificity of PGS of sIUFD were 0. 71 and 0. 72. The cut-off value was 72 points. Conclusions The PGS of sIUFD has good reliability, validity and sensitivity. It has an important clinical value on assessing the sad mood of sIUFD.
