近10年心律失常用药规律分析

摘　　要：目的 :从心律失常总体用药、缓慢性、快速性和整体用药的差别以及不同地域用药差别的角度,探讨心律失常的中医用药规律,指导临床用药。方法:对2005年1月1日至2014年12月31日10年间中国知网数据库(CNKI)和万方数据库收录的中医药治疗心律失常的相关文献进行全面检索,以频数统计为主,归纳分析心律失常的用药规律。结果:符合条件的文献共336篇,涉及全国29个省市地区,共用药192味,分为19大类,3 523频次。整体单味药使用频数最高的依次为甘草、丹参、麦冬、黄芪、桂枝等,缓慢性心律失常使用温热性药物的频数较高,快速性心律失常使用苦寒类药物的频数较高;整体药类以补虚药、活血化瘀药、解表药、清热药和安神药为主,缓慢性心律失常使用温里药和补阳药的频数较高,快速性心律失常清热药的使用频数较高。整体单味药用药前10味在不同地区间频率也有所差异。结论:快速性和缓慢性心律失常单味药的使用和药物分类情况分别和总体用药有较大差异,不同地区间的用药情况也有所不同。Objective：To investigate the medication rules of arrhythmia from the aspects of overall medication,the difference among bradyarrhythmia,tachyarrhythmia and overall medication,and the different regions to guide clinical medication. Methods ：A comprehensive search of the TCM treatment of arrhythmia published in CNKI and Wanfang database from Jan. 1st 2005 to Dec. 31st 2014 was carried out to analyze the med- ication rules of TCM treatment of arrhythmia by the frequency analysis. Results：Totally 336 articles were enrolled,covering 29 regions and 192 herbs which were divided into 19 categories,total appearance frequency of 3 523. Herbs with the highest frequency in the overall single medication included ：Licorice,Salvia, Ophiopogon,Astragalus and Cassia Twig. The highest frequency herbs in treating bradyarrhythmia were herbs warm and heat in nature. The highest frequency herbs in treating tachyarrhythmia were herbs cold in nature and bitter in taste. The overall medication mainly included：herbs of tonifying the deficiency,promoting the blood circulation to remove the blood stasis,expelling the exogenous pathogenic factors and calming the mind. Herbs of warming the interior and tonifying Yang had higher frequency in treating bradyarrhythmia while herbs of reducing heat had higher frequency in treating tachyarrhythmia. The first 10 overall single medications were different in different regions. Conclusion：The usage of single herb and herbal classification of bradyarrhythmia and tachyarrhythmia were respectively different from the overall medication,and the herbal usage between different regions were also different.
