肛周脓肿多重耐药菌感染危险因素分析

摘　　要：为探讨多重耐药菌（MDRMs）感染所致肛周脓肿的相关危险因素，选择2014年6月至2015年10月收治的肛周脓肿手术患者138例，根据脓液细菌培养结果，分离出MDRMs者32例为观察组，其余106例为对照组。比较2组患者的临床资料。结果显示，在138例肛周脓肿患者中，有32例患者分离出MDRMs，感染率为23．2％。观察组与对照组患者在性别、年龄、吸烟史、饮酒史、排便情况方面差异无统计学意义（P〉0．05）。2组在合并糖尿病、既往使用抗菌药物史、抗菌药物种类、疗程及使用第三代头孢菌素方面差异有统计学意义（P〈0．05）。经多因素Logistic回归分析显示，发生MDRMs感染与合并糖尿病、使用抗菌药物史、抗菌药物种类、疗程及使用第三代头孢菌素有关。结果表明，糖尿病、抗菌药物使用史、抗菌药物种类、疗程及使用第三代头孢菌素情况与肛周脓肿患者发生MDRMs感染密切相关。In order to explore the relevant risk factors of multi-drug resistant microorganisms（MDRMs） infection inducing perianal abscess,authors selected 138 cases of perianal abscess who had received surgery from 2014-06 to 2015-10 in author＇s hospital,according to the bacteria culture results of pus separated out 32 cases with MDRMs as observation group,rest 106 cases as control one,then compared the both group＇s clinical data.As results, the infective rate of MDRMs was 23.2 % （32/138） ;in patient＇s sex, age smoking history, drink history and the status on defecation there was no statistical difference between the two groups（ P 〉0.05）;but in with or without diabetes, previous used anti-bacterial drugs and their kinds, treatment course,as well the status on using the 3rd generation cephalosporins there was statistical difference between the two groups （P 〈 0.05）; multi-variation Logistic regression analysis showed the relativity of MDRMs infection and concurrent diabetes, the history of using anti-bactrial drugs, the drug＇s kinds, treatment course,as well using the 3rd generation cephalosporins.Results show that MDRMs infection induced perianal abscess is closely related to concurrent diabetes, the using history kinds and treatment course of anti-bacterial drugs,as well using the 3rd generation cephalosporins.
