围绝经期综合征应激性生活事件差异的影响因素分析

摘　　要：目的探讨围绝经期综合征妇女应激性生活事件差异的影响因素。方法对200例伴有围绝经期综合征的女性进行艾森克人格量表(EPQ)、生活事件量表(LES)、社会支持量表(SSRS)、汉密尔顿抑郁量表(HAMD),并检测血雌二醇激素(E2)及促卵泡刺激素(FSH)水平。结果根据生活事件(LES)总分是否大于20分划分为高应激组(46例)、低应激组(154例)。高应激组的外周雌二醇(E2)水平较低应激组低(t=-3.11,P<0.01)。而高应激组的卵泡刺激素(FSH)水平、LES得分、HAMD得分及神经质因子得分(EPQ-N)均比低应激组高(t=2.50,P<0.01;t=38.57,P<0.01;t=3.99,P<0.01;t=3.99,P<0.01)。高、低应激组二元Logistic分析:EPQ(N)、HAMD的回归系数β分别为0.151、0.045,P<0.01;模型拟合性:对数似然值为187.56,Nagelkerke R^2为0.20。结论影响围绝经综合征应激生活事件差异的因素包括神经质人格、抑郁状态,有必要制定个体化的精神心理咨询及干预策略以降低应激生活事件对围绝经期妇女情绪的影响。Objective To explore the influencing factors of stress life events in perimenopausal women. Methods 200 women with perimenopausal syndrome were investigated by using Eysenck Personality Questionnaire（ EPQ）,Life Events Scale（ LES）,Social Support Rating Scale（ SSRS）,Hamilton Depression scale（ HAMD）,and serum levels of estrogen（ E2） and follicle stimulating hormone（ FSH） were also be tested. Results The cases were divided into high stress group（ 46 cases） and low stress group（ 154cases） on the basis of LES scores（ higher or lower than 20 score）. The levels of estradiol（ E2） in high stress group were lower than that in low stress group（ t =- 3. 11,P 〈0. 01）. The levels of follicle stimulating hormone（ FSH）,stressful life events,HAMD and EPQ-N were higher in high stress group than those in low stress group（ t = 2. 50,P 〈0. 01; t = 38. 57,P 〈0. 01; t = 3. 99,P 〈0. 01; t =3. 99,P〈 0. 01）. The Logistic analysis showed that the beta regression coefficients of EPQ-N and HAMD were 0. 151 and 0. 045,and the differences were statistical significance（ P〈 0. 01）; log likelihood value of the model fit was 187. 56,and the Nagelkerke R^2 was 0. 20. Conclusion The influencing factors of stressful life events of perimenopausal syndrome included the neurotic personality and depression,it is necessary to develop individual mental and psychological counseling and intervention strategies to reduce the adverse of stressful events in perimenopausal women.
