慢性荨麻疹患者血清金黄色葡萄球菌超抗原特异性IgE检测及其意义

摘　　要：目的检测慢性荨麻疹(CU)患者血清金黄色葡萄球菌(简称金葡菌)超抗原肠毒素A(SEA)、肠毒素B(SEB)和中毒性休克综合征毒素(TssT-1)的特异性IgE(sIgE),分析该超抗原sIgE与慢性荨麻疹相关性,探讨其发病机制。方法采用酶联免疫吸附试验(Enzyme-linked immunosorbent assay,ELISA)检测63例慢性荨麻疹患者和60例正常对照组血清金葡菌超抗原SEA、SEB和TssT-1的sIgE水平,分别进行金葡菌超抗原(SEA、SEB、TssT-1和SEA/SEB/TssT-1)sIgE比较,并将其浓度与病情程度评分(UAS)进行相关性分析。结果 CU患者血清中金葡菌超抗原(SEB、TssT-1和SEA/SEB/TssT-1)sIgE水平均高于正常对照组,差异有统计学意义(P<0.05);金葡菌超抗原(SEA、SEB和SEA/SEB/TssT-1)sIgE水平分别与CU病情轻中重度和荨麻疹活动评分(Urticaria activity score,UAS)呈正相关(P<0.05)。结论慢性荨麻疹患者血清中金葡菌肠毒素A、肠毒素B和TssT-1 sIgE水平升高,可能与其慢性病程及严重程度有关。Objective In order to detect the serum special immunoglobulin E （IgE） to the staphylococcal superantigens staphylococcal enterotoxin A （SEA）, staphylococcal enterotoxin B （SEB）, and toxic shock syndrome toxin （TSST）-I in chronic urticaria （CU） patients, and to analyze association of specific IgE to staphylococcal superantigens with the CU and explore the pathogenesis. Methods The serum levels of special IgE to the staphylococcal superantigens （SEA, SEB and TSST-1） were determined by enzyme-linked immunosorbent assay （ELISA） in 63 patients with CU and 60 normal controls （NC）. The relationship between the serum slgE levels and the urticaria severity was analyzed using linear correlation. Results The serum slgE levels to staphylococcal superantigens in CU were＇significantly higher than those in NC （P〈0.05）. There was a positive correlation between the serum slgE levels to the staphylococcal superantigens （SEA, SEB and SEA or SEB or TssT-1 ） and the disease severity （P〈0.05） and UAS （P〈0.05）. Conclusion The higher serum slgE levels to the staphylococcal superantigens （SEA, SEB and TSST-1） might be implicated with the chronic pathogenesis and the urticaria severity in CU patients.
