逍遥散加减联合果酸治疗肝郁气滞型黄褐斑的疗效观察

摘　　要：目的观察逍遥散加减联合果酸治疗肝郁气滞型黄褐斑的临床疗效。方法将中医辨证为肝郁气滞型黄褐斑的患者92例随机分为3组:A组32例,逍遥散加减联合果酸治疗,B组32例,单用逍遥散加减治疗,C组28例,单用果酸治疗,治疗8周后进行疗效判定,停止治疗后进行随访。结果治疗8周后,A组总有效率84.38%,B组总有效率53.13%,C组总有效率60.71%,三者比较差异有统计学意义(P<0.05)。3组皮损治疗后积分均较治疗前有所下降,A组下降更明显。A组与B组中医证候疗效的有效率分别为75%和68.75%,两者差异无统计学意义(P>0.05)。3组复发率分别为6.25%,8.7%,16.67%。结论逍遥散加减联合果酸治疗肝郁气滞型黄褐斑,行之有效,临床值得推广应用。Objective In order to investigate the clinical efficacy of modified Xiaoyao powder combined with glycolic acid in treating melasma of syndrome of liver-qi stagnation. Methods Ninty-two patients with melasma of syndrome of liver-qi stagnation were divided into three groups. Thirty-two patients in group A were treated with modified Xiaoyao powder combined with glycolic acid, 32 patients in group B were treated with modified Xiaoyao powder only, and at the same time 28 patients in group C were treated with glycolic acid only. Efficacy was evaluated at the baseline and at the end of eight week treatment. Cases were followed up after the treatment. Results After treatment for eight weeks, the total effective rate of group A was 84.38%, which was much higher than that of group B （53.13%） and group C （60.71%）, and there were significant differences among the three two groups （P〈0.05）. Meanwhile, after treatment, the scores of lesions decreased in all three groups, and the change was greatest in group A. Furthermore, the effective rates of the TCM syndrome in group A and group B were 75% and 68.75% respectively, and there was no significant difference between the two groups （P〉0.05）. Recurrence rates of theses three groups were 6.25%, 8.7% and 16.67%. Conclusion Treatment of melasma by using modified Xiaoyao powder and glycolic acid is effective and worthy of clinical popularization and application.
