慢性阻塞性肺疾病急性加重患者有效排痰方案的建立及效果评价

摘　　要：目的探讨慢性阻塞性肺疾病急性加重(acute exacerbation of chronic obstructive pulmonary diseases,AECOPD)患者有效排痰方案的建立及实施效果。方法选取2013年65例AECOPD患者设为对照组,采取常规排痰管理模式;2014年64例AECOPD患者设为实验组,采取有效排痰措施,分别收集两组干预前及干预1周后肺功能指标、慢性阻塞性肺疾病评估测试(COPD Assessment Test,CAT)评分情况,干预后的有效排痰情况及平均住院日。结果在干预1周后,实验组在肺功能指标、CAT评分及有效排痰率均较对照组同期指标有明显改善(P<0.01);且实验组的平均住院日明显缩短(P<0.01)。结论有效排痰方案的建立可提升AECOPD患者有效排痰能力,改善患者的肺功能状态,提高生存质量,缩短平均住院日并降低医疗负担。Objective To explore the managenlent mode for effective sputum excretion of patients with acute exac- erbation of chronic obstructive pulmonary diseases（AECOPD） and analyze its clinical applieation value. Methods Totally 65 AECOPD patients admitted in 2013 were selected as the control group and received conventional sputum excretion management mode;64 AECOPD patients admitted in 2014 wereselected as the intervention group and received effective sputum excretion management mode with integrated medical care. For patients in both groups, lung function index before and 1 week after intervention,COPD Assessment Test（CAT） score,effective excretion rate after intervention and average duration of hospitalization were collected,respectively. Results After intervention for 1 week,lung function index,CAT score and effective excretion rate in the intervention group were significantly improved compared with those in the control group（P〈0.05）; moreover,average duration of hospitalization was obvi- ously shortened in the intervention group（P〈0.05）. Conclusion The multidisciplinai7 integrated medical care management mode based on pulmonary rehabilitation nurses for AECOPD patients could improve their lung function status and quality of life,shorten the average duration of hospitalization and reduce medical burden;it is worthy to be clinically popularized.
