摘 要:目的:以皮质酮肾阳虚动物模型为研究对象,采用正交设计研究多个针刺参数,探索影响针刺疗效的主次因素和针刺参数的最佳组合。方法:采用L9(3~4)正交表对4个参数(穴位、捻针角度、频率、时间)3水平安排试验,运用计算机控制针刺手法仪进行干预,通过ELISA检测血清T、垂体LH的水平和RT-PCR检测下丘脑中GnRH mRNA的表达,以比较上述4个参数的主次及每一参数不同水平的优劣。结果:对针刺疗效的影响由大到小依次为穴位、捻针角度、时间、频率,其中穴位是影响针刺疗效的主要因素,而捻针角度、频率、时间都是次要因素。结论:针刺治疗肾阳虚大鼠的最佳参数为A_3B_3C_2D_2,即针刺肾俞、捻针90°、频率120次/min、捻针60 s;本研究中穴位的选择对疗效的影响最大,其次为捻针角度,再次是捻针时间,影响最小的为捻针频率。Objective: To explore primary secondary factors that influences acupuncture effectiveness and the best combination of acupuncture parameters,orthogonal design was adopted to study several acupuncture factors. The rats model of kidney-Yang deficiency made by hydrocortisone as the study objects. Methods: 4 influential factors( i. e. selection of acupoints,twisting-rotating angle,twisting-rotating frequency and acupuncture time) plus 3 levels of each factor were arranged to L9( 3~4) orthogonal table. Kidney-Yang deficiency rats were treated by computer controlled acupuncture manipulation device. Serum T and pituitary LH were evaluated by ELISA method and hypothalamic GnRH mRNA expression by real time fluorescent quantitation PCR to illustrate the primary secondary factors and the good bad of different levels in each factor that could affect the acupuncture effectiveness. Results: Among 4 factors mentioned above,the order for influential factors from major to small was acupoints selection,twisting-rotating angle,acupuncure time and twisting-rotating frequency,among which,selection of acupoints was the major factors,twisting-rotating angle,twistingrotating frequency and acupuncture time all were secondary factors and. Conclusion: The best acupuncture parameters for treating kidney-Yang deficiency rats was combination of A3B3C2D2: selecting Shenshu acupoint( B23),90° of twistingrotating angle,120 tpm of twisting-rotating frequency and 60 s of acupuncture time; In this study,selection of acupoints became the biggest influential factor,the next was twisting-rotating angle,the secondary was acupuncture time,and the last one was twisting-rotating frequency.
