摘 要:目的比较声触诊成像定量技术(VTIQ)与声触诊定量技术(VTQ)鉴别乳腺良恶性肿瘤的价值。方法随机选择经病理证实的104例患者,共120枚乳腺肿瘤作为研究对象。120枚乳腺肿瘤中包括43枚恶性肿瘤和77枚良性肿瘤。分别在VTIQ及VTQ模式下测量肿瘤内部多组横向剪切波速度(SWV),比较两者的诊断效能。结果 VTIQ测量成功率达100%,而VTQ成功率84%(P<0.05)。VTIQ测量乳腺良性、恶性肿瘤SWV平均值分别为(3.73±1.02)m/s和(5.76±1.11)m/s;VTQ测量乳腺良性、恶性肿瘤SWV平均值分别为(3.84±0.68)m/s和(5.03±0.92)m/s。两种技术所测乳腺恶性肿瘤SWV值均明显高于乳腺良性肿瘤(P<0.01)。若VTIQ和VTQ诊断阈值分别取4.34m/s和4.05m/s时,诊断效能最佳,其诊断恶性肿瘤的灵敏度、特异度及符合率分别为91.7%、85.7%和87.1%;83.3%、72.7%和75.2%。其中,VTIQ特异度及符合率高于VTQ(P<0.05)。结论 VTIQ诊断乳腺良恶性肿瘤优于VTQ。Objective To compare diagnostic value of virtual touch imaging quantification (VTIQ) technique and virtual touch quantification (VTQ) in the differential diagnosis between benign and malignant breast lesions. Methods The imaging of 120 breast lesions in 104 patients confirmed by pathology were retrospectively analyzed. There were 43 malignant lesions and 77 benign lesions in 120 breast lesions. The average of shear wave velocity (SWV) values of the breast lesions were obtained from multiple SWV measurement under the VTIQ and VTQ mode. The diagnosis efficiency was compared between VTIQ and VTQ. Results The successful rate of VTIQ measurement was 100%, while that of VTQ measurement was 84% (P〈0.05). The SWV mean on VTIQ and VTQ of benign and malignant breast lesions Were (3.73±1.02)m/s, (5.76±1.11)m/s and (3.84±0.68)m/s, (5.03±0.92)m/s. There were significant differences between malignant and benign breast lesions in SWV mean on VTIQ and VTQ (P〈0.01). When the cut-off value of VTIQ and VTQ mean was 4.34 m/s and 4.05 m/s, the sensitivity, specificity and accuracy of diagnose malignant lesion conducted by VTIQ were of 91.7%, 85.7% and 87.1%, respectively, those of VTQ were 83.3%, 72.7% and 75.2%, respectively. The specificity and accuracy of VTIQ were higher than VTQ (P〈0.05). Conclusions The study proved that the VTIQ was better than VTQ in differential diagnosis of breast lesions.
