乳腺超声自动容积成像技术与钼靶诊断乳腺肿物的对比研究

摘　　要：目的 探讨乳腺超声自动容积成像技术（ABVS）与钼靶（MG）诊断乳腺肿物的应用价值。方法 利用ABVS及MG对138例女性患者共170枚乳腺肿物进行检查,以病理结果为金标准,对比评价ABVS、MG以及两者结合其各自的诊断效能。结果ABVS诊断乳腺肿物的灵敏度为98．0％、特异度为76．5％、准确度为89．4％；MG诊断乳腺肿物的灵敏度为72．5％、特异度为77．9％、准确度为74．7％；两者结合诊断乳腺肿物的灵敏度为98．0％、特异度为77．9％、准确度为90．0％。ABVS诊断乳腺肿物的灵敏度和准确度高于MG（χ^2=22．32、12．76，P〈0．05），两者结合诊断乳腺肿物的灵敏度和准确度高于单用MG（χ^2=24．04、15．36，P〈0．05）。ABVS、MG、两者结合的ROC曲线下面积分别为0．946、0．756、0．955。ABVS、MG乳腺病灶检出率分别为99％（168／170）、72％（123／170），ABVS乳腺病灶检出率高于MG（χ^2=41．33，P〈0．05）。结论ABVS相比MG在诊断乳腺肿物中具有较高的灵敏度和准确度，具有良好的临床应用价值。Objective To compare the value of automated breast volume scanner （ABVS） and mammography （MG）in diagnosing breast lesions. Methods 138 women patients with 170 breast lesions were identified by ABVS and MG with the standard of pathology. The diagnostic efficiency of ABVS, MG and the combination of two examinations were calculated. Results The sensitivity, specificity, accuracy rate of ABVS were 98.0%, 76.5%, 89.4%, respectively; which of MG were 72.5%, 77.9%, 74.7%, respectively; which of ABVS combined with MG were 98.0%, 77.9%, 90.0%. The sensitivity and accuracy rate of ABVS for diagnosing breast lesions were significantly higher than those of MG （χ^2= 22.32,12.76, P〈0.05）. The sensitivity and accuracy rate of ABVS combined with MG for diagnosing breast lesions were significantly higher than those of MG（χ^2= 24.04, 15.36, P〈0.05）. The area under the ROC curve of ABVS, MG and the combination of two examinations were 0. 946,0. 756 and 0. 955. The detection rate of ABVS and MG were 99%（168/170） and 72%（123/170）. The detection rate of ABVS were higher than that of MG（χ^2= 41.33, P〈0.05）. Conclusions ABVS has a higher sensitivity and accuracy rate than MG in the diagnosis of breast lesions. ABVS has an effective clinical application value.
