肾脏转移性肿瘤的超声诊断

摘　　要：目的探讨肾脏转移性肿瘤的超声特点及诊断价值。方法回顾性分析我院27例经手术、穿刺活检病理证实或临床有明确原发病灶并随访证实的肾转移瘤的超声表现。结果 27例肾转移共累及33侧肾脏,单侧受累21例,单灶19例,弥漫性受累2例,73.7%(14/19)的单发病灶发生于肾脏两极;双侧受累6例,均为多灶。超声扫查发现受累肾脏22例;漏诊11例,漏诊率33.3%。肾转移瘤声像图上多表现为形态不规则(13/22),边界不清(15/22),局部向肾外凸出生长(13/22),回声高于肾皮质回声或与肾皮质回声接近(20/22),内回声不均(18/22),可有囊变(12/22)。肺癌和食管癌来源的肾脏转移瘤声像图表现无明显统计学差异(P>0.05)。结论超声在肾转移瘤的检出及诊断上具有一定局限性,对有原发肿瘤病史伴肾脏新发病灶的患者,应警惕转移瘤的可能。Objective To investigate the diagnostic value of ultrasound in thyroid cancer in children. Methods The ultrasonographie features of 27 patients with renal metastases confirmed by biopsy or surgical pathology or follow up were reviewed. Results A total 33 cases of the kidney were identified in the 27 patients. 19 of these 33 cases were both unilateral and solitary; 2 were unilateral and diffuse, and the rest 12 cases were bilateral and multiple. 73.7% （14/19） of solitary lesions were located at either pole of the kidneys. 22 cases were detected by ultrasound and 11 ca- ses were missed. The rate of missed diagnosis was 33.3-. The renal metastases were more often irregular shape（]3/ 22）, ill defined（15/22）, exophytic（13/22）, higher or close to the echo of the renal cortex（20-22）, heterogenous（18/ 22）, with or without cystic degeneration（12/22）. There was no significant difference in the sonographic features of renal metastases derived from lung cancer and esophageal cancer. Conclusions In the detection and diagnosis of renal metastasis, ultrasound has certain limitations. Thus, as for the new renal lesion in patients with a history of primary tumor, we should be vigilant the possibility of metastasis.
