经皮椎间孔镜TESSYS技术治疗腰椎间盘突出症的临床研究

摘　　要：目的探讨经皮椎间孔镜TESSYS技术治疗腰椎间突出症的临床经验及疗效。方法通过分析2014年3月至2015年12月应用经皮椎间孔镜TESSYS技术进行手术治疗30例腰椎间盘突出症患者的临床资料,包括L_（3~4）2例,L_（4~5）18例,L_（5~S_110例;采用Oswestry功能障碍指数（ODI）、疼痛视觉模拟评分（VAS评分）和Macnab评价标准对患者的临床疗效进行评定。结果 30例患者术后均未发生感染、硬膜囊及神经根受损等并发症,术前ODI为（32.4±9.8）%,术后7日ODI为（11.4±4.6）%,术后3月ODI为（3.8±2.6）%,差异有统计学意义（P〈0.05）;术前VAS评分为（7.8±1.6）分,术后7日VAS评分为（3.4±1.4）分,术后3月VAS评分为（1.4±0.8）分,差异有统计学意义（P〈0.05）;Macnab评价结果：优25例,良5例,优良率为100%。证明所有患者临床症状均得到显著的改善。结论经皮椎间孔镜TESSYS技术在治疗腰椎间盘突出症中具有创伤小、术后恢复快、疗效良好的优点,是值得临床推广的安全、有效的治疗腰椎间盘突出症的方法。Objective： To explore the clinical efficacy and clinical experience of percutaneous transforaminal endoscopic technique with transforaminal endoscopic spinesystem（TESSYS）in treatment of lumbar disc hernia- tion（LDH）. Methods From Mar 2014 - Dec 2015, 30 patients suffering from lumbar disc herniation and treat- ed with TESSYS were enrolled in this study. In these 30 cases, 2 were L2--3, 18 were L4--5, 10 were L5-- S1. Clinical efficacy was evaluated by the visualanalogue score （VAS）, Oswestry disability index （ODI） and modified MacNab criteria. Results No complications such as scleromeninx rupture and nerve root damage were observed in all patients. The mean ODI of Preoperative were（32.4 ± 9.8） %, The mean ODI or seven days after the operation were（11. 4 ± 4.6） %, The mean ODI of three months after the operation were（3. 8 ± 2.6） %, The difference was statistically significant（P 〈 0.05）, The mean VAS of Preoperative were（32.4± 9.8） %, The mean VAS of seven days after the operation were（11. 4 ± 4.6） %, The mean VAS of three months after the op- eration were（3.8 ± 2.6）%, The difference was statistically significant（P 〈 0.05）, According to improved Mac- nab criteria, there were excellent in 25 cases, good in 5 cases. The rate of excellence was 100 %. Conclusion THESSYS percutaneous transforaminal endoscopic discectomy has lots of advantages such as minimal invasion and quick recovery, so it is a safe and effective method for clinical treatment of lumbar disc herniation.
