摘 要:目的 :探讨双能量CT(dual energy CT,DECT)成像在痛风患者筛查中的初步应用及诊断价值。方法 :选取我院因急性关节疼痛就诊患者122例,其中经临床明确诊断痛风患者98例,分析其成像特点与临床资料,对照组为非痛风患者24例。所有患者双足、双手均行DECT非增强扫描,薄层重组后将原始数据传至MMWP工作站选择GOUT软件行图像后处理。运用痛风识别技术分析DECT诊断痛风的总体敏感性及特异性,比较痛风患者和非痛风患者尿酸盐沉积部位和数目的差异。结果:2组性别及年龄比较,差异均无统计学意义(P_(年龄)=0.236,P_(性别)=0.613);DECT对痛风诊断的总体敏感性及特异性分别为91.84%、87.50%;痛风组检出率与对照组相比,差异有统计学意义(P=0.000)。结论:双能量CT痛风识别技术是一种新的成像诊断方法,具有高敏感性、非侵入性、可重复性的优势,对尿酸盐结晶沉积有很好的显示能力,对痛风性关节炎患者有较高的检出率,可作为痛风筛选的常规检查项目,具有较高的临床应用价值。Objective:To study the preliminary application of dual energy CT imaging in the screening of patients with gout,e-valuate the early diagnostic value in gout. Methods:To select the first hospital in qiqihar city for a total of 122 patients with acute joint pain clinic,to pass through the clinical diagnosis of 98 cases of gout patient,by the hospital ethics committee ap-proval of patients included in the study,analyze the imaging features and clinical data,and the control group of 24 cases of gout patients,all patients' feet/hands do DECT non enhanced scan,thin layer after restructuring would transmit the original data to choose MMWP workstation Dual Energy gout software,image post-processing,gout recognition analysis DECT overall sensitivity and specificity of diagnosis of gout,compare gout patients and patients with uric acid salt deposition part number and differ-ence;Using t test and χ^2 test,Fisher exact probability statistical analysis. Results:comparing two groups of gender and age,there was no statistically significant difference (Page=0.236,Pgender=0.613);DECT overall sensitivity and specificity for the diagnosis of gout was 91.84% and 87.50% respectively;Gout group detection rate compared with the control group,with significant difference sta tistically significant (P=0.000). Conclusion:Dual-source CT gout identification technology is a new method of imaging diagnosis, with high sensitivity,non-invasive and repeatability. DECT has a good ability to show urate crystal deposition, and has a higher detection rate in patients with gouty arthritis. DECT can be used as a routine inspection project of gout screening and has high clinical application value.
