摘  要:目的探讨micro RNA-146a在辅助诊断系统性红斑狼疮(systemic lupus erythematosus,SLE)中的价值。方法利用免疫印迹法、间接免疫荧光法、茎环RT实时荧光定量PCR(RT-q PCR)检测2014年9月-2015年2月在我院内分泌科住院的33例SLE患者、21例非SLE自身免疫性疾病对照组患者和24例健康体检对照组人血浆中抗可提取性核抗原(extractable nuclear antigen,ENA)、抗核抗体(antinuclear antibody,ANA)和micro RNA-146a的表达水平,比较3组间检测指标的差异;SLE患者稳定期与活动期micro RNA-146a的表达差异,micro RNA-146a与ANA检测水平的相关性。结果 SLE患者活动期micro RNA-146a相对表达量(relative expression,RE)高于稳定期(276.24±255.93 vs 102.76±59.55,P<0.05);SLE组micro RNA-146a的RE为223.67±229.31,高于疾病对照组(10.38±4.49)和正常对照组(4.68±3.18)(P<0.01),两对照组间差异无统计学意义;micro RNA-146a的表达量与ANA滴度无相关性(r=0.043、P=0.801)。结论 micro RNA-146a可以作为一个SLE的潜在诊断标记物,在判断SLE病情的严重程度、监测疾病的发生发展及恢复状况时有参考意义。Objective To investigate the role and clinical value of micro RNA-146 a in auxiliary diagnosis of systemic lupus erythematosus(SLE). Methods Western Blot method, indirect immunofluorescence method, stem ring RT real-time fluorescence quantitative PCR(RT-q PCR) were used to detect anti-ENA(Extractable Nuclear Antigen), ANA(antinuclear antibody) and micro RNA-146 a level in plasma of 33 SLE patients, 21 non-SLE patients in disease control group and 24 healthy cases in control group, the differences of detection index between three groups were compared; micro RNA-146 a expression differences of patients in stable or active stage, and the correlation between detection level of ANA and micro RNA-146 a were also compared. Results For SLE patients, the relative expression(RE) of micro RNA-146 a in active subgroup was higher than inactive subgroup [(276.24±255.93) vs(102.76±59.55), P〈0.05]; and the micro RNA-146 a RE of SLE group was higher than disease control group and normal control group [(223.67±229.31) vs(10.38±4.49),(4.68±3.18), P〈0.01], while there was no statistical difference between two control groups. The correlation coefficient of level of micro RNA-146 a and ANA was r=0.043, P=0.801.Conclusion micro RNA-146 a can be used as a potential diagnostic marker of SLE, and it has important value in judging the severity of SLE and monitoring disease onset, development and the status of recovery.
