清肝降脂汤治疗脾虚痰瘀互结型非酒精性脂肪性肝炎临床观察

摘　　要：目的观察清肝降脂汤治疗非酒精性脂肪性肝炎的临床疗效。方法将125例脾虚痰瘀互结型非酒精性脂肪性肝炎患者随机分为治疗组(62例)和对照组(63例);治疗组服用清肝降脂汤,对照组口服易善复片和当飞利肝宁胶囊,疗程为24周。观察治疗前后两组患者中医证候积分、肝功能、血脂、体质量、BMI、肝脏B超变化情况。结果 1治疗后两组中医证候积分均较治疗前显著下降(P<0.01),但组间差异没有统计学意义(P>0.05);2治疗后两组ALT、AST、TC、TG、γ-GT水平均较治疗前显著下降(P<0.01),但组间差异无统计学意义(P>0.05);3治疗后,两组体质量和BMI均较治疗前显著下降(P<0.05,P<0.01),但组间差异没有统计学意义(P>0.05);4治疗后两组B超病情均有显著改善(P<0.01),且治疗组改善明显优于对照组(P<0.01)。结论清肝降脂汤治疗脾虚痰瘀互结型非酒精性脂肪性肝炎能显著改善患者临床症状和肝功能,降低脂肪肝严重程度。Objective To observe the clinical efficacy of ＂Qinggan Jiangzhi Decoction＂ in the treatment of nonalcoholic steatohepatitis with spleen-deficiency and phlegm-stasis. Methods One hundred and twenty-five subjects were randomly divided into two groups： the treatment group in which 62 cases were treated with ＂Qinggan Jiangzhi Decoction＂ and the control group in which 63 cases were treated with polyene phosphatidylcholine tablet and Dangfei Liganning Capsule, with a course of 24 weeks. The syndrome score of TCM, liver function, blood lipid, body weight, BMI, B-ultrasonic results of the liver were observed and compared before and after treatment. Results ①Mter treatment, the syndrome scores of TCM significantly decreased in both groups compared with before treatment （P 〈0.01 ）, but with no statistical difference between the two groups（P 〉0.05 ） ;②After treatment, the levels of ALT, AST, TC, TG and γ-GT significantly decreased in both groups compared with before treatment（ P 〈0.01 ）, but with no statistical difference between the two groups（ P 〉0.05 ） ;③After treatment, the body weight and BMI significantly decreased in both groups compared with before treatment（P 〈0.05, P 〈0.01 ）, but with no statistical difference between the two groups （P 〉0.05 ） ;④After treatment, the B-ultrasonic results of the liver were improved in both groups （P 〈0.01 ）, and the efficacy in the treatment group was better than that in the control group（P 〈0.01 ）. Conclusion ＂Qinggan Jiangzhi Decoction＂ could be used in the treatment of nonalcoholic steatohepatitis with spleen-deficiency and phlegm-stasis, which significantly improved clinical symptoms and liver functions, especially alleviates the severity of fatty liver.
