腰后路改良PLIF椎间融合术治疗老年腰椎管狭窄合并下腰椎不稳的临床研究

摘　　要：目的 研究腰后路改良PLIF椎间融合术治疗老年腰椎管狭窄合并下腰椎不稳的效果。方法 选取2010年6月~2015年1月我院收治的35例有腰椎管狭窄合并下腰椎不稳的老年患者,将其随机分为对照组17例、观察组18例。对照组采用传统PLIF对患者治疗,观察组采用改良后PLIF对患者进行治疗,比较两组患者的临床治疗效果。结果 观察组患者总有效率达94.4%,显著高于对照组的64.7%（P〈0.05）;观察组术后融合率高于对照组,术中临近节段退变率明显低于对照组（P〈0.05）。结论 改良后PLIF椎间融合术治疗老年腰椎管狭窄合并下腰椎不稳,获得同等减压效果的同时保留部分后方结构,椎体稳定性更好,提高了椎间融合率,临床效果更好,值得临床广泛推广应用。Objective To study the clinical effect of application of modified posterior lumbar interbody fusion （PLIF） in the treatment of elderly patients with lumbar spinal stenosis complicated with lower lumbar spine instability.Methods 35 cases of elderly patients with lumbar spinal stenosis complicated with lower lumbar spine who were treated in the hospital from June 2010 to January 2015 were selected and randomly divided into the two groups with 17 cases in ob- servation group and 18 cases in control group.Control group was treated with conventional PLIF,while observation group was treated with modified PLIF.The clinical effect of the two groups was compared.Results The total effective rate of observation group treated with the modified PLIF was 94.4%,significantly higher than that of control group（64.7%） （P〈 O.05）.The postoperative fusion rate of observation group was higher than that of control group and the adjacent segment degeneration rate of observation group decreased significantly in comparison with control group （P〈0.OS）.Conclusion Application of modified PLIF for elderly patients with lumbar spinal stenosis complicated with lumbar instability obtains equivalent decompression effects and also retains the rear part of the structure.h achieves better vertebral stability and clinical effects,and improves the intervertebral fusion rate,which deserves wider clinical application.
