摘 要:目的探讨支持(support)、技巧(skill)、自信(self-confidence)即3S理念教育模式在留置涤纶套双腔导管行维持性血液透析(血透)患者中的应用效果。方法将120例维持性血透患者随机分为两组,对照组60例实施传统的健康教育,研究组60例应用3S理念健康教育模式进行干预,观察两组干预前后疾病相关知识与导管维护技能的掌握情况;比较两组干预后6个月内导管功能不良、导管感染、血栓并发症发生率。结果干预前两组疾病知识与导管维护技能得分比较差异无统计学意义(P>0.05),干预后两组疾病知识与技能得分均较前提高(P<0.01),研究组提高更明显(P<0.01)。干预后6个月内研究组总并发症发生率为28.33%(17/60);对照组总并发症发生率为46.67%(28/60)。两组并发症发生率比较差异有统计学意义(χ2=4.3022,P=0.0381),研究组并发症发生率较低。结论 3S理念教育模式能提高维持性血透患者疾病知识与导管维护技能水平,树立战胜疾病信心,减少并发症的发生,值得推广应用。Objective To explore the application effect of 3S ( support, skill and self-conlidence) concept of health education mode in maintenance hemodialysis patients with indwelling dacron cuff duallumen catheter. Methods 120 mainte-nance hemodialysis patients with indwelling dacron cuff duallumen catheter were randomly divided into research group (60 cases) and control group (60 cases).The control group were given traditional health education,while the research group were inten^ened by 3S concept of health education mode.Mastering level of disease-related knowledge and skills of catheter mainte-nance before and after intenention were obsened between the two groups.The incidence of catheter dysfunction, catheter in-fection and thrombus complications between the two groups alter 6 months of inten^ention were compared. Results Before in- ten^ention,the difference of the two groups5 scores in disease-related knowledge and catheter maintenance skills was not sta-tistically significant( P〉0.05) ,and those scores alter intenention obviously increased( P〈0.01) ,and the improvements of the research group were more visible than that of the control group( P〈0.01) .After 6 months of inten^ention, the total incidence of complications of the research group was 28.33%(17/60) , while that of the control group was 46.67% ( 28/60) .In addition, difference of incidence of complications between the two groups was statistically significant( χ^2 = 4.3022, 0. 0381) , andthat of the research group was significantly lower. Conclusion 3S concept of health education mode can improve the maste-ring levels of disease-related knowledge and catheter maintenance skills of maintenance hemodialysis patients with indwelling dacron cuff dual-lumen catheter.lt is worthy of popularizing because it can help patients set up confidence to overcome disea-ses and reduce occurrence of complications.
