胃管引导改良法在放置三代喉罩中的应用

摘　　要：目的：观察胃管引导改良法置入三代喉罩的临床可行性,安全性和有效性。方法选择18~80岁,ASA Ⅰ~Ⅱ级非腹腔镜、非胃肠道择期手术患者100例,随机分为两组,全身麻醉后分别采用传统徒手法（ C组）和胃管引导改良法（S组）置入三代喉罩,比较两组喉罩置入成功所需时间、一次性成功率、拔出喉罩后前端是否染血,患者咽喉部不适评分以及术中反流误吸例数。结果改良组喉罩置入一次性成功率明显高于传统组,差异有统计学意义（P〈0．05）；拔出喉罩后前端染血率明显低于传统组,差异有统计学意义（P〈0．05）。两组置入喉罩成功所需时间差异无统计学意义（P〉0．05）,术后咽喉部不适评分差异无统计学意义（P〉0．05）。结论采用胃管引导改良法置入三代喉罩较传统徒手法更容易,术后相关并发症更少,值得临床推广应用。Objective To investigate the clinical feasibility,safety and efficacy of the application of an improved method of the stomach guide tube in the placement of PLMA. Methods 100 patients who were18 to 80 years old,ASAⅠ~Ⅱlevel with-out laparoscopic and gastrointestinal surgery were selected and randomly divided into two groups. After general anesthesia,respec-tively using the traditional manual method（group C）and improved method of stomach guide tube（group S）to place PLMA. Then comparing the two groups in terms of the successful insertion time of PLMA,one-time success rate,the leading edge was bloody or not after unplugging PLMA,discomfort score of patients’ throat and cases number of regurgitation and aspiration during the surgery. Results The one-time success rate of PLMA insertion in the improved group（ group S） was significantly higher than the traditional group（group C）,the difference was statistically significant（P〈0. 05）;After removing PLMA,the rate that the leading edge was bloody in the improved group（ group S） was significantly lower than the traditional group（ group C） ,the difference was statistically significant（P〈0. 05）. The difference in the successful insertion time of PLMA between the two groups was no statistically signifi-cant（P〉0. 05）. Postoperative throat discomfort rates between the two groups showed no significant difference（P〉0. 05）. Conclu-sion The improved method of the stomach guide tube in the placement of PLMA was easier than the traditional manual method, which had less postoperative complications and was worthy of clinical application.
