后路经伤椎短节段椎弓根钉固定联合植骨融合治疗胸腰段脊椎骨折的疗效评价

摘　　要：目的评价后路经伤椎短节段椎弓根钉固定联合植骨融合治疗胸腰段脊椎骨折的疗效。方法将120例胸腰段脊椎骨折患者随机分为两组,对照组(n=60)行跨伤椎短节段椎弓根钉固定联合植骨融合治疗,观察组(n=60)行经伤椎短节段椎弓根钉固定联合植骨融合治疗,比较两组患者的手术时间、术中出血量、住院时间、VAS评分、JOA评分,以及随访12个月的椎体压缩程度及矢状面Cobb角矫正度丢失情况。结果两组患者的手术时间、术中出血量、住院时间、VAS评分、JOA评分差异无显著性(P均>0.05);观察组随访12月的椎体压缩程度及矢状面Cobb角矫正度丢失均优于对照组(P均<0.05)。结论后路经伤椎短节段椎弓根钉固定联合植骨融合治疗胸腰段脊椎骨折的疗效显著,且术后伤椎椎体高度保持和减少术后矢状面Cobb角丢失方面的远期疗效优于跨伤椎治疗。Objective To evaluate the curative effect of posterior vertebral short segment pedicle screw fixation combined wit^ fusion in treatment of thoracolumbar spine fractures. Methods 120 patients with thoracicolumbar vertebral fracture were randomly divided into two groups , control group （ n = 60） and observation group （ n = 60） , patients in control group were applied with cross injured vertebral short segmental pedicle screw fixation combined with bone graft fusion for treatment, and patients in observation group were treated with vertebral short segmental pedicle screw fixation combined bone graft fusion for treatment. The operating time, amount of blood loss, length of hospital stay, VAS scores, JOA scores and the degree of correction of Cobb angle in patients of these two groups were compared. Results The difference in operating time, amount of intraoperative blood loss, duration of hospital stay, VAS scores and JOA scores between patients of these 2 groups was not significant （ P 〉 0. 05）. Conclusion The curative effect of posterior vertebral short segment pedicle screw fixation combined with bone graft fusion in treat-ment of thoracolumbar spine fractures is distinct, and the maintenance of height of vertebral bodies after surgery and the reducion of postoperative sagittal Cobb angle loss was superior to that of treatment by across the injured vertebrae.
