妊娠合并糖尿病孕妇同时并发子痫前期因素及母儿不良影响

摘　　要：目的:探讨妊娠合并糖尿病孕妇同时并发子痫前期危险因素及对母婴的不良影响。方法:选取2013年2月至2015年5月东莞市虎门中医院和东莞市第五人民医院收治的53例妊娠合并糖尿病同时并发子痫前期患者为观察组,选取同时本院收治的68例妊娠合并糖尿病患者为对照组,比较两组患者年龄、产次、发病孕周、体质量指数(BMI)及母婴并发症。结果:观察组患者年龄、产次、BMI均大于对照组,且发病孕周小于对照组,组间比较,差异具有统计学意义(P<0.05)。观察组并发症中产后出血发生率高于对照组,组间比较,差异具有统计学意义(P<0.05)。观察组围产儿并发症中早产儿、新生儿窒息及新生儿低血糖发生率均高于对照组,组间比较,差异具有统计学意义(P<0.05)。结论:孕妇年龄、产次、体质量影响妊娠合并糖尿病同时并发子痫前期发病,可加重患者血管病变,影响子宫、胎盘,可增加母婴不良妊娠结局的发生。Objective To investigate the effects of gestational diabetes while pregnant risk factors for pre-eclampsia and for female children bad. Method Select 53 cases of gestational diabetes concurrent preeclampsia study group in February 2013 May 2015 in our hospital, our hospital while selecting patients with diabetes mellitus 68 cases of pregnancy as the control group, compare the two groups of patients in age, parity, gestational age of onset, body mass index （BMI） and mother of children complications. Result The patient＇s age, parity, BMI study group were higher than the control group, the incidence of gestational age than the control group [（32.5 ± 3.1） years vs. （31.0 ± 2.8） years, （1.5 ± 0.4） times vs.（0.9 ± 0.3） times, （31.6 ± 3.6） kg / m2 vs. （29.7 ± 3.5） kg / m2, （25.4 ± 7.8） weeks vs. （28.7 ± 7.1） weeks, P 〈0.05]. Mother complications incidence of postpartum hemorrhage study group was higher [20.75% vs. 4.41%, P 〈0.05]. Per±natal complications in preterm children, neonatal asphyxia and neonatal hypoglycemia incidence study group were higher [than 4.41% 24.53%, 15.09% than 1.47%, 16.98% than 2.94%, P 〈0.05]. Conclusion Maternal age, parity, gestational diabetes affect weight concurrent preeclampsia, may increase the patient vascular disease, affect the uterus, placenta and increase maternal adverse pregnancy outcomes, clinical strengthen management, improve maternal prognosis.
