摘　　要：目的探讨自体脂肪注射移植隆乳术的技巧与远期效果。方法自1997年11月至2015年12月，采用注射器吸脂、静置沉淀＋离心法纯化脂肪，遵循3M原则与少量多次原则，为367例患者实施注射隆乳术；分析总结手术技巧及远期随访结果。结果本组患者注射1—4次，每次每侧注射量为30～200ml。随访1个月至16年，双乳均较术前明显增大，外观丰满、挺拔，1例术后16年患者在乳腺后间隙形成厚约2cm，具有独立包膜的脂肪垫；29例出现局部硬结，其中6例行手术切除，病理显示钙化结节；其余病例经按摩、热敷等非手术治疗自行消退；所有患者无血肿、感染、液化坏死等并发症发生。结论遵照3M原则与少量多次的注射原则，自体脂肪移植隆乳术是一种效果肯定的隆乳方法。Objective To study the technique and long-term curative effect of autologous fat grafting for breast augmentation. Methods From November 1997 to December 2015, autologous lipochondria were harves- ted with syringe liposuction and refined with static settlement and centrifugation, and then were transplanted into 367 patients for breast augmentation by ＂3M＂ principle and in ＂small quantities, multiple times＂. The opera- tive technique and results of long-term follow-up were summarized and analyzed. Results Each patient re- ceived fat injection for 1 to 4 time. The injectived dose was 30 to 200 ml on each side, each time. All patients were followed up from 1 month to 16 years. Compared with pre-operation, the bilateral breasts increased obvi- ously with plump, tall and straight appearances. A 2 cm thickness fat pad within its own capsule appeared in the retromammary space in one patient at 16 years postoperatively. A local scleroma was found in 29 cases. A- mong them, the scleroma in 6 cases was excised and showed calcification scleroma on pathological examination and the others were treated by non-operative treatments, such as massage and hot compress. No hematoma, infection, liquefaction or necrosis occurred in all cases. Conclusion When used with ＂ 3M＂ principle and ＂small quantity injections, multiple times＂, autologous fat grafting for breast augmentation has a positive out- come.
