高龄住院老年人餐后低血压的临床观察及护理干预

摘　　要：目的观察110例高龄住院老人餐后低血压（PPH）的发生情况，分析可能影响PPH的临床情况，以期探讨老年人PPH的护理干预策略，进一步加强预见性护理以减少PPH的发生及其带来的不良后果。方法选择110例住院老年人，观察三餐前2min及三餐后15min、30min、60min、120min的血压变化，筛查出PPH的患者，并记录低血压相关的临床症状，收集患者的基础疾病和服药情况等临床资料。结果110例老年人中，PPH患者56例，PPH发生率为50．91％；PPH的患者三餐分布情况为早餐50例（89．28％）、午餐33例（58．93％）、晚餐38例（67．86％）；PPH患者观测血压当天有相关症状者8例，既往有症状者37例。结论高龄老人中PPH发生率高，适当的健康教育和预见性护理策略对预防PPH的发生及其带来的意外伤害具有重要临床意义。Objective To observe the occurrence of postprandial hypotension （PPH） of 110 elderly patients, and analyze the possible clinical situations related to PPH to explore the nursing intervention strategies for PPH for the elderly and further strengthen the predictive nursing care to reduce the occurrence of PPH and its adverse conse- quences. Methods Totally 110 cases of hospitalized elderly were chosen as the research objects. And the changes of blood pressures 2 minutes before meals, and 15 minutes, 30 minutes, 60 minutes, 120 minutes after three meals were measured and observed. Patients with PPH were screened. The hypotension-related clinical symptoms were recorded, and the basic clinical information for disease and medicine-taking of patients were collected. Results Among the 110 cases, 56 patients suffered from PPH, the incidence rate of PPH was 50. 91%. The distribution of PPH patients was as following： 50 cases at breakfast time （89.28%）, 33 cases at lunch time （58.93%）, and 38 cases at dinner time （67.86%） . 8 cases presented hypotension-related clinical symptoms on the day of observation, and 37 cases had showed hypotension-related clinical symptoms before enrollment. Conclusion There is a high incidence rate of PPH a- mong elderly patients, and the appropriate health education and predictive nursing intervention strategies show critical clinical significance for oreventing the occurrence of PPH and the accidental iniurv.
